Dude I've gone from hemmorrhoids to prostate issues to pelvic floor issues and CPPS and stretched the hell out of my hamstrings to thinking it was sciatica and because of your videos I finally know what I'm suffering from. Oh my gosh! A doctor who actually knows how to diagnose! As an ex sports car mechanic who was known as a good diagnostician I can't say that I have been overly impressed by the diagnostic skills of the many doctors I’ve gone to over the past 60 years. In fact I've often found myself thinking, "It's a damn good thing that my doctor isn't a car mechanic because the side of the highway would be littered with inoperative cars! Really great to watch a video produced by a doctor who can put two facts together and come up with a solution! Thanks!

Wow! This is so clear and I I can finally make sense of my pain and the proper exercises to look for to help me! Thank You! I have wasted so much time and money up to this point with no answers and only temporary relief I was never given this info nor any separate testing to confirm things. Thank you for showing how to diagnose and explaining as you go. Very helpful and informative. As it turns out, its none of the issues. So I have to continue to diagnose. I do believe its a hammy issue, but only seems to aggravate with load bearing movement.

This is an awesome demonstration to help diagnose my issue. I am pretty sure mine is a high hamstring tendinopathy. I have been suffering for months and have started doing the bridge type exercises and quit stretching and doing any dead lift type movement. Thank you for your great information.

Hi Doc., Thank you for this video. This is invaluable... I have been struggling with the NHS in the UK to pin point my problem and get to the root of it. I am still waiting for them to diagnose. After chancing on your video, I can tell myself that I have a high hamstring issue indeed. Crystal clear. I have been struggling with this for the past 4 year intermittently. During the process, I have got my X-ray done for the hip, Sonogram to check if it is hernia( as had pain shooting to my crotch area). Thanks once again!

Hey Catherine! Welcome-thanks for watching. Look up Askling L protocol. That being said, I'd caution you to try and DIY this- as there is a bit of an art to tendon rehab. Too much and you risk making it worse... too little and not enough stimulus for change. You'll want to understand progressive overload principles, when to push into pain vs avoid... the difference between isometrics and isotonics/when to make the shift. So many things to understand here... Not to scare you... but rather educate you on some things to consider. As much as I've coached people throughout these issues in the past... it's not my primary focus at the moment, so I would urge you to look locally.

As the pain I feel now isn't too bad yet, I want to try to mimic what I did with my PT then. But if it doesn't get better in the next 2-3 weeks, I'll surely get professional help!

Thanks for all of this great information!!!! My issue is kind of complicated, because, I do have back pain, I have sacroiliac changes( arthritis) I also have hip bursitis (bilateral) gluteus minimus tears, gluteus medius tears and labrum tears( bilateral) but, started having back of thigh pain(now and than) about four years ago, and than, about two months ago, did a lot of walking, and, since than, the back of thigh pain( bilateral) hasn’t stopped!!!! There’s some “sits(?)” bone pain on the left side, but, mostly, middle of thigh(back) pain and incredible stiffness. MRI’s show tendinopathy, 2 years ago, but, until 2 months ago, it wasn’t so much of a problem. So, back issues, SI joint changes, hip bursitis, hamstring tendinopathy, which is causing all of the pain and stiffness behind BOTH THIGHS, in your opinion?!!!

This is so helpful, thank you for your thoroughness. I took a hard fall on a rock while hiking and have a partial tear/hamstring tendinitis and herniated disc on L5-S1 thought is touching sciatic nerve. Then to add some fun I have a bad bone bruise on my tibia that isn't healing. So .... my whole left leg hurts and I can't tell what is causing what. After watching this I think it's my disc issues that are causing my issues even though I don't feel anything in low back, just glutes.

Technically anything is possible here... meaning, there have been some cases of what they term "hamstring syndrome" whereby the hamstring tendons seem to cause irritation/compression of the sciatic nerve as it passes by. I'm not sure of the prevalence on this (but likely super low). Instead, I'd take the one that seems most involved... optimized your healing environment (to reduce stress on the irritated structure - if any and regardless of which one)- then treat the movements that are yucky. I figured it was a hamstring tendon since I did a bunch of hamstring curls earlier, but this definitely helped clarify especially as a newbie to frequent exercise! Thanks 👍 definitely a weird sensation, it's a real pain in the butt! 😆

Hey! Sure thing 👍. Is possible… but usually the back is what causes “hamstring” to feel like it’s also issue (it’s usually the sciatic nerve not hammy). Back problem is upstream and can make it seem like there are many other problems (hammy issues included) occurring downstream! Make sense? Hi Doc, Firstly having watched numerous youtube, none come near your explanations. your brilliant flow charts are great to narrow things down.. however having done most of the tests I am still stuck, it started 2 months ago.. my crippling pain seems to be mostly in ankle and outside of calf muscles ( sometimes around hip area brought about by a 'load' ie standing or walking ( which i can only do for 2-3mins tops. sitting in a car now has same calf and ankle pain.. it leads to me understanding it is some form of sciatica but its origins i am lost on... can you shed any light on the matter ( God bless you for all the help you have given to suffers like me)

Hard to know what is what now.Have had this butt problem for 2 years now,been to chiro,acupuncture,Bowan therapy and physio.Found physio best.X-rays,MRIs and ultrasounds.Went to surgeon at first as hips sore,he said hips ok but had some nerve damage in L leg which I wasn’t surprised at as had back problem back in 2017which caused sciatica- laminectomy was done. pain free until this problem began.Been doing stretch exercisesfir 6 months to stretch Piriformas muscle stopped a month or so ago as seemed that the stretches weren’t helping anymore.After watching your video I am going to try some of the exercises that shorten the muscle and see if that works.Lots of information from your videos,great stuff.x

Great breakdown of syptoms!! I've been diagnosed with hamstring tendinopathy but you mentioned the driving psin is sciatica! When I drive it's has a burning deep pain feeling. You mentioned driving pain is sciatica. Can this pain also occur with hamstring problems? Thanks

Hey- thanks for watching. Hamstring issues can cause buttock pain while driving (a form of compression)- but its local to the sits bone/attachment site of the hamstring.

Thank you for the detailed video. Based on the diagnostic tests, it looks like I'm clearly in the hammy camp. However, when walking, or jogging slow (not fast), I sometimes get an odd dull weakening of the whole leg, like the leg is hinting at collapsing. Also, a similar ache down the whole leg occurs sometimes when sleeping on either side. Any tips on what to check, do next would be appreciated. Thank you!

This is a fantastic explanation to help me figure out what has been plaguing me for 4 months now. It's definitely not a back problem and I'm pretty sure while it's not sciatica, I do get some stabbing pains you described while doing the HHT tests. none of the sciatica tests (30-70 degree SLR, etc.) cause pain. Once I get to 90 degrees and even flex the toes, it's intense. While I do feel it down my leg, I think it's referred pain to the nerve, but the pain right at my sit bones is definitely a major sign for me that it's HHT. I can stand all day long at work without pain, but once I sit down for 30 minutes at my desk, the pain is intense. I have tried to sit on a lacrosse ball while on long drives but it doesn't really help long term. I still have to stop once an hour. Once I stand up, the pain is 100% gone within a matter of seconds. Question now is what is your treatment plan for someone with HHT and some slight sciatic nerve referred pain?

I've watched this twice to be certain and I believe I have high hamstring issues. It's been 4.5 weeks since I've run, I've been walking and doing high hamstring strengthing exercises. I'm also trying laser therapy to hopefully accelerate the healing. I'm just wondering how much longer I will feel this hamstring pressure so I can run again soon.

after a year of mris and xrays and pt, all they found just mild foraminal stenosis but dr believes that is not creating lateral hip pain and glute pain. pain normally happens when i sit or lie on my hips. it sucks. this seems like it could be it since I no longer have lbp a year later-mostly butt pain sitting.

Really useful video, thank you! I've had left-sided buttock pain for ~6 months, worse when walking up hills & worst when running. I can reproduce the pain by lying on my back, legs straight and digging my left heel into the floor as hard as possible. Less so with your exercise ball test, with the affected leg bent. Would you say that this still suggests HHT? Is the bent leg significant? I am 6'5" with very long legs, so I guess the load with a straight leg is that much more.

@@drcharliejohnson Yes, this started after trying to get back into running, I think some time in November. Probably didn't warm up and/or did too much too soon. I had (self-diagnosed) ischial bursitis in early October - couldn't sit comfortably on my left buttock for almost 1 week, then got completely better really quickly. Laid off the running for several weeks but was walking regularly. I will check out your guide! Thank you 🙂

i have some high glute pain lateral and slightly superior to the SI joint. occasionally it is non symptomatic but often it gives me trouble walking. ive never had shooting pain down the leg and i think its from an old deadlifting injury. is there any way to diagnose this type of thing from home?

Hi there. Please help I’ve been battling sciatica on and off for 6 years now 😞 for me, the sitting hunched over on a cushion straight leg raise had ZERO symptoms. However the straight left raise lying down is excruciatingly sore. My physio is unsure whether it is a hamstring tendon, glute tendon, or disc problem. Thoughts? Much appreciated content 🙏

Agree whaterver you said .I struggled one year with right high hamstring pain ...But every time i go to doctors they tell me same story it is due to back as i have some mild disc bulge also in MRI and all the sciatica producing motions did not create any pain,No back pain ,only sitting on chair make bum feel sore and very irritating.Back physiotherapy was not helping so they suggest me to go for steriod injections and maybe later they suggest surgery of back and that will be disaster.Sometimes correct diagnoses is very important to road to recovery.Now i am focusing on hamstring rehab and my problem is improving very much with in few days.

I've had a VERY problematic pain like this for the last week. It's localized to the left side, down low in the cheek, and is a HOT, sharp pain that doesn't radiate down my leg and has no accompanying tingling sensations. The pain only occurs when I exert my left leg by stepping up on to something like a ladder or stool, or when squatting down. Occasionally it bothers me when I sit, but only in the act of moving to a seated position quickly (i.e., sitting down on a bed or something). Standing or basic walking causes no pain. I'm going to contact my doctor this week about it. I'm going to test some of these exercises to see if it's a nerve or muscle/connective thing. Any thoughts? This is helpful. Thanks!!

Hello. Where are you based? I am experiencing butt and upper thigh pain for a very long time. I’m having an injection in my piriformis muscle soon and i’m hoping this will help so i can start strengthening the muscles in my leg.

I would describe HHT as following: if you rest the symptomps settle down you feel good. If you load with hamstring exercise, it will flare up the next day(s). If it fits to you, you have HHT.

Have no back pain. I did the laying down with the ball and I get a tickling sensation. Sometimes I get a sharp pain at the top of hamstring during the day

Can’t your upper hamstring injury lay down scar tissue that wraps around your sciatic nerve, causing sciatica issues along with your hamstring pain sitting or stretching?

Good question Carolyn! Yes, we call this hamstring syndrome... but it's virtually impossible to tell without exploratory surgery. So, it all comes back to what we can control... motion and our mind... and then using these tools and a process to resolve pain!

I have 3 herniated disk on MRI but my worse pain is not my back. I have chronic back pain. I know the day my hamstring started hurting and now it is worse. They think it's sciatic pain based on my MRI. My pain is worse when I sit down.

I think I have HHT, based on watching this video. When I place the toes of my good leg onto the heel of my bad leg to remove my boots, my pain is very sharp all the way down the back of my leg. Would this be expected with HHT?

I have osteoarthritis in both hips. Right hip rep. Left one getting ready to rep. Avascular necrosis in both. Now I feel like knots live in my buttocks. No pain going down legs except big toe and leg hip feels different. Please tell me what to do

I have a feeling of fishing lines yanked behind my legs and tingling in my feet soles and heels Feels like screws in pelvis is loose and causing all this. It especially comes on with adduction on adduction machine Lemme know what you think Will appreciate your help Thank you

Luckily there are no screws holding the pelvis together. So good news- no issue there. If machine causes symptoms (could be for many reasons- back arching, too much resistance etc) hold on doing them… stop. I feel from touch massage a hard lump of scar tissue right at my high hamstring. This flares up from time to time which then feels like it is pressing on my nerve. This is debilitating and I cannot put pressure on that foot on the floor so end up in bed. What to do please ??

My pain is very random, sometimes I bed over or stand up and my but goes completely numb and pain sometimes I almost pass out. It feels like a cramp. It’s not like I just have pain every time I bend over or sit I don’t get it.

I haven’t done much for treatment because I had been dealing with IBS until about a year ago I switched diets and have fixed most of that and I thought maybe the pain/cramps were related to my IBS but I have started stretching is about my only treatment. I also work in lawn care and landscaping not sure that activity would be considered good for it but I am a pretty active person. Glad to know at least maybe I am on the right track of figuring this out thanks.

@@drcharliejohnson - My wife bought me a hot tub for my birthday, so I sit in there and try to stretch my hamstring. Take Advil (I know it's not good for me) and Tylenol does nothing. Having trouble walking very far. Dale.

Well, this left me even more confused. At first when I saw this I wondered how I'd missed this video but as soon as it started I thought I had misdiagnosed sciatica and instead have high hamstring. Like another guy said here, I can walk and stand all day with little pain, I have no real back pain but sitting in a car seat is ungodly painful-well, it WAS but its a little less now that I've been doign strength training. But when i ran through all these exercises NONE of them created pain, despite the fact I had just driven and really felt it. There is a vague feeling that shoots up my middle back, but only very vague and only very occasionally. When I sleep on my back it wakes me early and I walk it off, but noticed that it usually goes down significantly with wall squats. As soon as I use that muscle, the pain diminishes. But if I push on my ass cheek thats bad, I can feel like a bruise way in there. But when I used to sit I always had the feeling that I was sitting right on that bone and wanted to build up that ass muscle. Either way it seems taht strength training is the way to go, although I've been doing a lot of that and now have to do considerable stretching because the muscles always feel tight, which is also bad.
