This is completely me ! I have also been through so many people who haven’t helped. I’m also a Pilates teacher so am completely baffled as I seem to do all the right things but this shows it must be something I’m unconsciously doing repeatedly. I hate sitting too long I sleep the same way as the lady . I have too much range of motion in my hip external internal rotation. My hips feel tight and loose at the same time. Pain down front and sometimes down to front of foot. Physio has just tried dry needling for first time

I like out of box thinking...yet my situation is driving is a freedom for me. For me it about finding a mindful position to sit in that doesn't engage the piriformis.

What do I have then? I don’t have any back pain, only pain in my left buttock when I sit and sleep on my left side. I did all test and I didn’t have any pain while performing them.

Yes! very common that back issues refer to buttock and are misdiagnosed as PS! Look upstream first! Thanks for watching Carl.

@@Gabi-ud1cq hey! Thanks! Good question. Opposite of left forward bend would be back bend to the right 👍. You can do this in various positions… lying on belly and achieving a back bend to right. Starting on all 4s and sinking into a back bend right. Standing back bend right… kneeling… or even lying on belly and kicking right leg up/back can accomplish a back bendy type motion to the right. Many motion/position combos to explore to help you unlock your yucky motions! So, don’t get discouraged if one way doesn’t work… where there’s a will, there’s a way and it simply takes testing and a bit of a process to find what will work for you! 👊🏼 Let me know if you get stuck or need help.

As soon as I watched this video I became aware how I was standing at the sink and doing dishes and folding towels. When you were standing at the sink and showed how you could stand and relax the back muscles verses leaning over and tightening the back muscles it helped me understand how I have been putting a lot of strain on my back while doing regular household chores. I have some bad habits to overcome. I really appreciate your videos.

Donuts just put pressure on coxcyx. A travel pillow or a nursing pillow with an open side, will relieve pressure.

Hi, I have a question about side plank. I have pain in my right glute and I’m working on strengthening it. Should my right leg be on the bottom or left leg on the bottom during these exercises?

@@Gymmotivation_by_Harshit good question. Honestly, it’s less about “strengthening” and more so about finding what feels best. So, if strength is the main goal- the right leg would be on the bottom. If pain relief is your goal then pick the side plank that feels painfree and work that to start. Make sense?

@ thanks for your response doc. I discovered this lengthened piriformis syndrome while doing heavy deadlifts, and now I can’t deadlift or squat anymore because I feel pain in my right glute. My goal is to fully recover so that I can return to powerlifting movements at my fullest capacity. What would you recommend I do for that?

Best advice I can give is hit the Jacuzzi it' does miracles better than any stretches and massages

Great video, thank you! I have several of the ongoing issues. SI joint, piriformis and hip bursitis. Lots of stretching and lidocaine patches. 🤷🏻‍♀️

@@waynec9444 have reviewed over 10k clinical back, butt, sciatica pain cases. Used to be an adjunct professor at USC and Penn State Hershey Medical Center. Residency trained and orthopedic board specialized- top 1% of PTs in the USA alone.

I have been dealing with the pain (already less) in my leg and weakness for 5 weeks. My muscles are weak - hamstring and in front side of lower leg - foot drop. I have tried to strengthen foot wit dorsi flexion with no successful. After this vid. I have realised that I must let it heal and no aggravate those areas.

Watched the video and did all the tests and I am more confused than ever! All sorts of clues but none that really follow any logical path on your flow charts. Too bad you are in the USA or I would book an appointment. It's helpful to know which tests to bring to my own physiotherapist though!

I’d gone to PT for PS. I stretched my Piriformas with success. However, the pain returned in a few weeks, but my usual stretching exercises weren’t helping. I found this video and tried to “shorten” the Piriformis as Charlie explained and tried his exercises. Within a couple of days, I felt relief from the pain. Thanks.

No, stay as active as possible. Avoid a few specific stretches and poking the painful area. Be mindful of ways that these positions which stretch the area may show up in life for you.. and then modify those activities positions. Thats the big idea.

This was so helpful. I’ve been wondering if my migrating pain could possibly be TMS related as there is no explanation for it. It’s nice to hear from a PT that it is absolutely a possibility!

I just saw a comment from myself 4 years ago. These last 2 weeks it has returned. Sitting is killing me. A stinging isolated pain in my right upper butt. It hurts like hell. Everything seems like a momentary relief and it comes back a few momente later. My right buttock muscle feels super tight whenever i sitt. Any idea?

Light bulb moment! I now finally understand why walking is when I pain. Seeing you demonstrate the kick back Is actually bending your back. That is my most yucky movement but never applied it to a walking motion! Understanding the why is powerful knowledge. Thank you for this demonstration!

If im on pain/anti inflammatory medication could these symptoms be masked and harder to diagnose. Ive had left glute pain for about a week after doing an irregular excercise woke up yesterday morning with excruciating pain in lower back and left side of buttox .But ive been taking defein and now just extremely tight glute and tingling/numbness in my left foot ?

I get sharp pains in the hip that I am having trouble getting diagnosed. When I rotate left leg outwards, it feels like something is pinching. When I do the second test in this video and pull knee towards opposite shoulder, no pain other than a stretch. But when I release the stretch, I feel the pinch in the hip. I can stand on the affected leg by itself for 30+ seconds without pain so I don’t think it’s DGS or GTPS. Any thoughts what might cause pinching at the hip on outward leg rotation?
