Hi Sam! Based on the Philippine Commission on Women, it's not required (those who want to keep their maiden names are free to do so). For reference, please see below: “a married woman has the option, but not a duty, to use the surname of the husband.” Therefore, upon marriage, married women have the option to continuously use her maiden name or: Her maiden first name and surname and add her husband’s surname; or Her maiden first name and her husband’s surname; or Her husband’s full name, but prefixing a word indicating that she is his wife, such as “Mrs.” Hope that helps, thank you!

Hi Avee, you have to go to your RDO po. As for your concern, they might ask for a marriage document that carries your maiden name (allowed naman po to keep your maiden name as per Article 370 of the Civil Code of the Philippines). Hope that helps, thank you! =)
