My Page 47 ‘LESSON ONE -Accented Notes’ - the page after ‘EXERCISE NINE’ seems to most closely resemble it?

To play all these ostinatos I did this: First you need to play all kinds of paradiddles (RLRR, LRLL, LRRL, RLLR) with ostinato, when you get used to all of them it's a piece of cake to play entire exercise.

What about the 1/8 note rest in bar 2? Do you play over that?

Yes, you play a double stroke aka a diddle. Check out time code 3:09 to clarify this concept. I hope this helps.
