Neighborhood Enrichment Team (NET) Program - Spring Gardening at Camelback Post Acute Care and Rehab

As part of a Centers for Medicare & Medicaid Services (CMS) nursing home effort, HSAG launched the Neighborhood Enrichment Team (NET) Program to facilitate community engagement and enhance the quality of life of nursing home residents. Through the NET Program, HSAG partnered with Camelback Post Acute Care and Rehab, in Phoenix, Arizona, and collaborated with community businesses and organizations to organize gardening activities for the residents. This video captures the collaborative efforts of volunteers and residents, and shares the joy of gardening and planting, fostering a deep sense of purpose and connection with the community.
