Excellent summary Dr C, I find such diagrams really helpful, and showing the interactions and feedback loops is excellent. You mentioned cerebral aneurysm. One aspect of h/t inadequately recognised imho, is aortic aneurysm, both abdominal and thoracic. High BP is a *short-term* danger to the aorta (BP peaks causing the aorta to fail), as well as long term. A recent paper shows a 4 fold increase in risk of aortic dissection from NBP of 110/75 to 150/90 - even 140/90 confers extra risk. Also important to control cholesterol as the aortic tissues suffer similarly to the smaller vessels. All of these can lead to Acute Aortic Syndrome - dissection, PAU, IMH. The big problem with AD is that it is usually asymptomatic up until a dissection, or rupture, a life changing or ending experience - unless a patient has, luckily, has an incidental finding of aneurysm. NICE have been lobbied to recognise the tighter BP control needed in aortopathy, but so far that's gone in the too hard tray, as has recognition of AD in A&E.

Thank you Dr Campbell. My husband was diagnosed with essential hypertension in late October 2019. He had previously (4 years previously) been told he had Graves Disease but he had not accepted treatment for it. Now he is on four medications, including one diuretic. Is insulin resistance a major challenge with hypertensive individuals? Would CGM monitoring news useful? To monitor his blood glucose and quarterly testing of his HbA1c. I’ve read some of Dr Joseph Krafts papers and listened to an interview with him and think hyper tension and hyper insulin are closely related. Thank you so much for these two presentations.

Dear Dr John, Do you have a video on pheochromocytoma? I’ve can’t find one by you about catecholamines and high BP, budd chari like syndrome from vena cava obstruction due to this cancer. It’s such a confusing cancer and I know only you could help me understand it. We have amazing specialists and did SRT which resolved ascites. However non removable with extensive metastatic spread so being managed with phenoxybenzamine well.

@@Campbellteaching Regarding acid reflux and GERD and the whole aisle at the local drug store devoted to antacids, what about the proposal that insufficient HCl production and insufficient digestive enzyme production are the culprits, so that neutralizing acidity is exactly wrong thinking?

Does being on a strict Low carbohydrates eating plan with moderate protein and healthy essential fats help hypertension? By lowering inflammation? Something Dr Sarah Hallberg mentioned in the Virta Health reports. Also something Dr David Unwin mentioned but they weren’t referring to hypertension but type 2 diabetes

Hi,dr.John! I like so much your videos. I have a little problem and i hope you can help me. At 16 i had my first panic attack and then for 3 days i had 1 another in each day.I went to the doctor which consult me(pulse 160 and 16/10 i think pressure).When i arrived there i felt so much anxiety. I did an ekg and it showed a 170 pulse and hipertension (i was very very stirred during it).He let me to calm down and my pulse drops to 130.He said it happens because of the hormones and i have to calm down myself. He let me for 2 years with metoprolol and other pills. Then for being he sure he gave me to do some blood test. He wasn.t surprised when the results came. My thiroyd hormone was 8.16 (double than normal maxim) and my thyroid hormones were normal. He said that all these are in my head and i have to "burn" the hormones.Now i.m 20 and at 19 i stopped the betablocker. If i stop the blocker for 1 day my heart rate where around 100 in rest. For 2 weeks that was my heart rate and then it drops to 80-85 in rest. Since then this is my resting pulse and feeling the same anxiety...My anxiety started only when i wanted go to blackboard or writing at school,at doctor and in effort it grew fast. Now my resting pulse is around 80-85 when i dont make movement or effort for some time but sometimes at effort it adjusts and my pulse decreases to 70-75. He also told me that he heard a murmur at auscultation and that he would disappear when he would pass this thing.I still can.t control myself for don.t start these episodes of tachycardia...Can you give me some advice on what to do to control these emotions?

Alex, The most basic mistaken psychiatry is to assume a condition is psychological when it might be physiological. Is there a good local physician who can do a full medical assessment on you? For example, you say your thyroid hormone is high. If you are hyperthyroid this should be diagnosed and treated accordingly.
