I've noticed other people saying the same, I think it may have something to do with where you are in the main story when you attempt this side mission. The video shows the puzzle attempted as soon as the mission was available, but I think if you try it in Act 3 it may be harder.

I had the same problem. 3-2-2 pattern, so I dropped the difficulty and reloaded the last checkpoint. The last checkpoint is right after you beat everyone so you just walk back over to the inhaler and examine it. This time, you will have the same puzzle, but different pieces, and you will actually be able to complete it. Hope this helps.

Jay _ yeah this is a game error because the pieces are backwards. I tried to see if there was an indication to flip them the right way but you can’t. They need to update and fix it. This is a glitch.
