@@Freecpapadvice Hi Jason. I bit the bullet and bought the device. I'm getting good suction at the start of the evening, but experiencing seal break as my tongue falls back later in the evening. Cant sleep on my back at all without this happening, and also occurring on my side unless head is facing a bit downwards. Seems like the suction weakens a bit throughout the night which I suspect is due to the saliva pouch filling up. Upping the intensity on the device doesnt increase suction, only the speed at which the motor reacts. Did you only use the standard mouthpiece that came with it, and any seal breaks for you? Promising device but not quite there yet I feel.

I am a long-time CPAP user. I tried iNap because I wanted to try a negative pressure therapy system and I have some concerns re: positive pressure pushing air into my lung. I like that iNap doesn't require a mask which that night was considered my first night of "freedom" from the mask. On the first night, I struggled to place the mouthpiece (also my tongue) in the right position. But the 2nd night onwards, I was able to put the mouthpiece and my tongue in the right position, and that was when the negative pressure worked well. I felt great the next morning due to the long sleep night. I don't think this will replace my CPAP, but it will be an excellent option when I travel. For sure, this will be an excellent system for those who can't comply with the mask.

I love the CPAP machine, it has literally changed my life, but, as you mentioned, camping is a difficulty, and this would be amazing if it works for me. Also wanted to say thanks for all you do, finding out I had sleep apnea terrified me, and you've made me feel so much better, you've helped me laugh about this illness, and I really appreciate you and your knowledge.

I used an oral appliance for over 10 years. Then had to go back to CPAP recently. The oral appliance worked but not as well as CPAP. MY AHI was 17 events an hour. The downside of an oral appliance is the cost. My dentist made it. $2000. And it pretty much messes up your jaw because it pushes the lower jaw forward. So it changes your bite. Which in turn affects eating and speech. The nightly problem with an oral appliance is drooling! Lots of drool. And waking up to wipe it off. Also the oral appliance doesn't always stay in position and might slip off the teeth. Well, nothing is perfect. Great video. Thanks.

Super interested, but between the cost of the doc appt plus the device and consumables, and that I'm a self-pay patient, I don't want to spend that chunk for something that seems to me should be about $500 (or less). Regardless, WTF does a doctor have to be involved? I know, I know... *rules*. Or as I like to rephrase it, "mandatory administrative activities of HIGHLY questionable value". This is the biggest aggravation (to me) about treating OSA.

Thanks Jason. Great and timely video. I'm extremely interested in this device and will let you know how it works for me. In the meantime, I offer this pre-evaluation WITHOUT having used the device yet. 1.Small 2.Lightweight 3. Quiet 4. Rechargeable 5.Low power use, only to initiate the seal. 6. Unobtrusive 7.Nothing on your face. 8.Unaffected by head movement, contact with bedding, etc. 9.No need to pressurize the airway or lungs. 10. Allows completely normal breathing. 11. No humidity or heating issues. 12. Non-invasive 13. Directly addresses the problematic tissues instead of trying to drag them around by mouthguards or MMA surgery. 14. If one needs combination therapy, the device doesn't rule that out. There's a lot to like. As an engineer, I think these people are on the right track.

Hi John, did you get the device ? how is it going?
@john99776
@john99776 2 жыл бұрын
@@lorenzoishere No I haven't. I've been reading a couple of studies about it carefully, and although it pulls on both the tongue and the palate to seal them together forward of the airway, it doesn't increase the space between the tongue base and the back of the throat. In fact it decreases that space slightly, even in people that it helps, due to the way it changes the overall shape of the tongue as it suctions it upward - the base of the tongue ends up a bit further back. I've had several palate surgeries, and a sedated endoscopy that I just had (to see if I qualify for Inspire device) shows that most of my apnea is caused by the tongue base. So the Inap may not help me much, if at all. And unless I take part in a study, there's the downside that if I use it exclusively I won't know how well it's working. I've also had a sort of breakthrough in the last week. My results with APAP have been all over the map - from the 30s AHI down to 2. Now, thanks to Jason, I've tried mouth taping and have been getting consistent results below 5. So even though I'm approved for Inspire, I may cancel the surgery. I'm not happy with the big battery and circuit in one's chest, or further messing about with the nerves in my tongue after already having the tongue suspension surgery (that didn't work). Both the Inspire and the Inap only seem to get people down to the 10 or so range, on average. If I can get 1s and 2s consitently with the APAP, this health improvement, even if subjectively I'm not attentive enough to sense during the day, can be read off the machine each morning and is probably (finally!) making a real difference to my health and longevity, so it's probably worth the hassle of being tied to keeping a machine with me. I'm also interested in the aerSleep, which suctions itself under your jaw and is undergoing testing now. But again, you'd need to be studied somehow to make sure how well it's working. For me, that device, from the animation on the website, looks like it would move the base of the tongue forward better than the Inap, and would be easier, less invasive and more comfortable to use. Even if I continue to use APAP, the aerSleep looks like it could be an ancillary device for clapping on when lying back to take a nap.

@@sathmath8544 I have a few dozen patients using the device. The "extra" data sleep docs have access to is not much more: total use, percentage of use with seal, days used >4 hours, (negative) pressure. The device can also be paired with 3rd party pulse oximeters, and the app will generate a nightly oxygen desaturation index (ODI). Since the device can't effectively measure airflow or obstruction, the ODI is a proxy for device efficacy, as imperfect as it is
@sathmath8544
@sathmath8544 2 жыл бұрын
@@wellrestedmd4619 It did sound from the interview that current data provided even to doctors is not very useful. Some companies are getting very advanced sleep and apnea data from o2ring so somnics can go that route with cheap accessory devices. Using just ODI is being lazy. Data to clinicians only is definitely not the right direction. Do your patients find any dentition discomfort as a result of the device? How do you track their treatment outcome with such limited data?
@wellrestedmd4619
@wellrestedmd4619 2 жыл бұрын
@@sathmath8544 Even limited data can be extremely useful, we don't need every option to be a perfect solution. The biggest problem in the management of sleep apnea is that around 90% of sufferers have never been formally diagnosed. The 2nd biggest problem is that among those who've been diagnosed, long term adherence to treatment is not much better than 30%. So as a clinician, two foundational pieces of information to guide care going forward is 1: has a diagnosis been established, and 2: is the most effective treatment being utilized. The limited data provided by the iNAP by the very mechanism of the device include whether it's turned on or not, and second whether a seal is achieved or not; there's not much else that can be derived from such a simple mechanism. An unused or underused treatment is not going to be effective, and if the iNAP isn't maintaining a seal during the vast majority of the use, it can't be helpful. So these two pieces of information from iNAP about use time and seal time are of prime importance. I would also be skeptical of consumer-grade data from something like the O2Ring - being required by law to "register" with the FDA has nothing to do with FDA authorization or approval, and Wellue has yet to publish any validation of their claims. As far as the mouthpiece, I've yet to hear from any of my patients that dental discomfort is an issue, and it hasn't been reported in the literature either.

I can see this becoming the default treatment. CPAP discontinuation rate is far too high for it to be considered the "gold standard", and those of us left using it are seldom satisfied. The main cons for iNAP seem to be the lack of viability for patients with congestion issues, the absence of information on patient viability and being difficult to ascertain the device's efficacy without a follow-up polysomnography. Another potential benefit would be, given that the device is only being used for a few minutes each night, it should theoretically last for a very long time. Yet it's being sold with a paltry 2-year warranty, compared to my CPAP that came with a 5 year one. Thus the question remains, whether it has been built to last, or built to fleece patients? It would also be good if it's possible to test this on a more severe patient, to compare nothing with iNAP with CPAP. No offense, but your OSA doesn't seem bad enough to make meaningful comparisons.
@Freecpapadvice
@Freecpapadvice 2 жыл бұрын
All great points. The only one that I really would disagree with is the CPAP discontinuation rate. With proper education and follow up it can be much higher. (what this channel is trying to improve) No offense taken. This sounds odd....but I am disappointed that my AHI was a low as it was without. I know it's higher than that. I'm using nothing right now because I strongly believe I had a "residual CPAP effect" from using iNAP. With nothing I felt REALLY clear on the night of testing....which just doesn't happen normally. The good news is that in 10 days with no therapy I'm starting to feel like/and sleep like shit again. So I'll retest myself and compare again. That said, I'd still like to see this on a moderate and severe OSA patient.

Jason, great video series. Time will tell if the product works well for those of us without severe OSA, but it's good to see there is some development in this space. The lack of data access is a big drawback and hopefully the manufacturer comes around. I have a few questions for you. 1. How is data sent to the physicians? Is it wireless, SD card, or a USB port on the device you have to bring with you? 2. If physicians are supposedly the only ones with access to the data, how will you be able to perform an analysis for patients who use this device? Thanks!
@Freecpapadvice

Thank you Ed. 1. It's cloud based. When it syncs with your phone app I believe the data is made available to your "provider". 2. Me personally provide analysis? I'd have to run a sleep study on them with the device in place. If increased negative pressures were to be tested a 2 night series could be run with comparison. $150 now, a 2 night test would be 4 times that. I could try to get creative to bring the price down, but it would still be much more expensive.

I use both a MAD and a bipap with a full face mask. I designed the MAD myself based on 3d scanned teeth data. After all soft tissue issues were resolved, it turned out I have a larynx disorder, I get laryngospasms at night. No amount of pressure can resolve that. I was considering tracheotomy, but then decided to pay a questionable plastic surgeon instead to do a custom cricothyrotomy. Long term it is considered controversial, but so much less invasive than tracheotomy. I'm currently designing vents, sockets, magnetic connectors and what not. Hopefully I'll be able to make it work permanently.

I have mild sleep apnea, although terrible insomnia because of it and will wake up about every 90 minutes when not using cpap. I have had cpap for 5 years and use for a period of a few months before I stop again. The problem I have with cpap is that while I will wake up less during the night, and my AHI reduces, I wake up feeling very tired. Even when I have leaks well controlled. I find that I get a deeper sleep without cpap even with waking up constantly. I think that my brain won't relax fully while the pressure is going. Recently I have tried an otc MAD to test before I buy an expensive one. I had some improvement but way too much TMJ and tooth pain. I actually have had good success with an otc tongue sleeve. The tongue hurts a little after but no significant jaw pain and I slept better than in a long time. This is why I think the iNAP may work for me, and I scheduled an appointment with my doctor to talk about a Rx.

The biggest positive that I see is that it continues running through power outages. Third thing I thought about and you mentioned it happened, but drooling and that sound like a suction machine. If you have allergies, or you have nose drainage it might be a little too much to suction and the device will be clogged.
