In the Injustice Mobile App he is classified as a meta human, so maybe he really has this appearance. I he mean, did actually die in the prequel comic book series, and now he is back, maybe his looks are an effect of his revival.

What's really interesting to me is that in the prequel comics, he's killed by the Joker before everything starts, so I'm interested to see how that comes into play. It almost looks like he might legitimately be undead or spectral, rather than just in costume or projecting someone's fears

He's not big all the time. When he activates his Venom is only when he grows in size, but this size Bane is actually accurate for his everyday look

Craig Furguson I think he uses the fear gas in his intro. So at first he is in his 'human' form and after spraying the fear gas he looks like a monster. The other player 'sees' him like that. That is also probably the reason Why the camera is shacking during the intro.
