A typical life-flight helicopter costs about $15 million dollars. Then add in the medical equipment and personnel, maintenance, fuel, and the outrageously expensive insurance a medi-vac chopper must carry, and $40k isn't an outrageous price, and nearly all private medical insurance in the US covers medically-necessary air transportation.
