I have a question - when you are splitting the prism for those large PDs squints, how should the position of the prism be placed on the fellow eye E.g. using your example of 100PD esotropia of RE, I will put 60 PD base out prism for the RE, but for the 40PD for the LE - should it be base out or base in? How should we go about deciding the orientation of the prism for the fellow eye? Thank yoU!

It's the same in other eye as well...base out both eyes in eso and base in both eyes in exo

If the left eye has an esotropia, as you had drawn , the image will be formed temporal to the fovea and not nasal.

because the muscle of the deviated eye is not paretic or restrictive ...it has equal strength to the fixating eye so there will be equal innervation and the deviations are equal

Mam, I have one doubt please confirm that while doing the alternate cover test when we find the perfect neutralization point the squinted eye will stay deviated behind the prism (where no fixation required) or it will reach to center ?
