It was eliminated first in the textbook version of this problem.

This is incorrectly done in the first step - A should have been eliminated first since they had zero first place votes. Please update.

why did the A votes get transfered to D and not C

This example of the IRV method is incorrect because "A" would be eliminated in Round 1 first because it had the least amount of first-choice votes (0), and then so on and so forth....

Wouldn't C be eliminated before D because it's smaller? Also A should be eliminated first?

What if there are 2 or 3 out of 4 candidates having won the same fewest votes in the first counting? In this case which candidate should be eliminated?

These are different names for the same method, so no difference :)
