Yes, this video is for Florida standards. You will not have two basins available for this skill, in most testing settings. For the exam, you can use two basins, if available. A better alternative: Use one basin with clean water, apply soap to the washcloth (never in the water in the basin to keep the rinse water clean) then set the soapy washcloth aside after use. This keeps you from having to remember which basin is clean/dirty and provides clean water for rinsing. Good luck!

The state exam does not grade on side rail usage because not all beds in healthcare have siderails (they are considered a restraint). Scooting the patient toward you on the bed before rolling them away from you keeps them in the middle of the bed after rolling, which promotes safety. Raising the bed is for your comfort and not graded (but you MUST lower it if you raise it for safety). You remain in the room after raising the bed so warn the patient when you wash your hands and get supplies.

No, it does not matter. The standard actually does not indicate the amount of times the catheter tubing needs to be cleaned. However, as an RN, I recognize the need for adequate infection control processes when a patient has an indwelling catheter to reduce the risk of UTI's and teach 4 strokes to ensure adequate cleaning. It also helps keep things consistent when training. But how many times you wipe is up to you on the exam. Good luck!

Side rails are not required to be used in the test, and in fact, the testing center may not even have side rails because they will not be present in all settings (they are illegal in certain settings, like assisted living facilities). You may use them if you wish, but do not leave them up at the end of the skill because they are considered a restraint. Good luck!

quick question for anyone who knows.Should you roll the patient/resident away from you as preformed in the video or toward you? In class we were taught to do it toward you and ask examiner to hold patient because its "safer" and im taking my test in about a week i want to know the current way for my test and for the future......

Ensure the catheter tubing is coiled on the bed before turning so that there is enough slack that it does not pull at the insertion site. Good luck!

You need to wash the catheter, then rinse the catheter (you cannot use the same washcloth to wash and rinse). Then you have to wash the skin around the catheter (peri-area) and rinse the peri-area (you cannot use the same washcloth to wash and rinse). I do not see how this skill can be done with 3 washcloths without cross-contamination. Good luck!

I was taught to clean the peri area sides first and then center last for 2015 standards because they said that you want to clean it from cleanest to dirtiest. Could I still clean the peri area like shown in the video and not get in trouble for doing it that way? I'm more comfortable with the way that it was shown in the video Thanks! :)

+Jasmine Hughery The checklist does not specify that you must clean from cleanest to dirtiest and you are using different areas of the washcloth to cleanse with each stroke using the leaves method, which decreases cross contamination (the whole point of clean to dirty). The area around the urethra really should be cleansed first to decrease the likelihood of introducing bacterial contamination into the urethra which could cause a UTI. This is the rationale for the method shown in the video. This method conforms to all current testing standards.

It is not required to flex the patient's knees for this skill, as that action will overly expose the most private area of the patient and does not promote privacy. You may do this if you wish (it is not graded) but if you were the patient, which would you feel more comfortable with? For routine cleaning, this position is more than adequate for cleaning the perineum without overly exposing the patient. Good luck.

Not for catheter care. The patient is not incontinent, so there is no need for an absorbent pad that will trap heat against the body and could cause rashes. You would leave a chux under the patient after peri-care to absorb the incontinent fluids. Good luck! Like our facebook page if this helped!

+Sean McNamara No. You cannot sign with CNA initials because you are not technically a CNA yet. Simply sign your name, first and last, as you would sign any signature. There are only 4 skills that require documentation: pulse, respirations, feeding and emptying drainage bag. All of these are simple "concrete" measurement documentations of numbers, not long narrative types of documentation. Usually, in a clinical setting, concrete numerical documentation of these measurements will not be signed. For the exam, you are signing the documentation not as a CNA, but as a testimony that this is the documentation you are recording for testing purposes (that way later you cannot come back and say "No, I told the evaluator that I got 14 on the respirations, not 20" since it is your reading in your handwriting and signed by you). Also do u know if prometric grades on side rails and raising and lowering the bed . Im confused on what part to actually touch the bed controls for contamination purposes . Obviously u want to touch bed controls before your hands are washed but i don't want to raise the bed then leave to go get supplies and wash my hands leaving patient in a high bed ?????

For this skill when i practiced in my class i made sure the tubing wasn't kinked but when i rolled her to the side the catheter tube was kind of tight looking like it was being pulled from the insertion on the manikin and i don't want that part to fail me on the test in case i might have catheter care on my skills part of the test . Any tips ?

Please review our newest skills videos for updated skills performance. But you should NEVER roll a patient toward you - this is a safety risk because gravity will always try to pull them onto their back because their weight distribution is along their backside. If their back is on the other side of the bed from you, you will not be able to stop them from rolling backwards (and possibly off the bed) because they are rolling away from you. +Sean McNamara You may clean either the labia first or the catheter tubing first. The test does not indicate which is to be cleaned first. The clinical skills checklist lists the checkpoint for cleaning the labia first, but does not specify that the labia must be cleaned first (they had to pick one to list first but that does not indicate that one specifically must be done before another. You may clean either area first, as you prefer. Thank you for your comment.

Turning the patient TOWARD you is not good body mechanics as it positions the patient's center of gravity away from you so that you lose control if they begin to fall. It also positions the patient's face in line with your crotch, never a good idea. Turning a patient away from you promotes safety and proper body mechanics. Good luck!
