I took my NYS CNA test March 22. This was one of my skills . Along with r.o.m to the left shoulder and pulse. I watched all her videos and it really helped. I haven't been in class since September 2018. I passed . I was super nervous, my anxiety high. Best thing to remember is introduce , explain , check I'd bracelt , pull curtain and wash hands ! Very big on that

How funny one day you will be a expert. I remember the first time I was a new CNA and I was assigned to the largest wing in a huge nursing home. I could tell that the Lady told to train me acted like it was a bad deal to train someone, so I said hey just tell me what to do I take orders. In pass down I learned that Wing-A had a massive UTI in almost all the female residents. SO me being a BSN student at the time I wondered if I could figure out why all the female residents have UTI's. SO we go in the 1st room and she shows me how to do Pericare! 1st she wipes FRONT TO BACK, then again FRONT TO BACK, and then more quickly FRONT TO BACK, FRONT TO BACK, FRONT TO BACK! I didn't want to correct an experienced ol' CNA because they tend to hate for no reason, and carry grudges, back stabbers...BUT after the 3rd resident I snapped and said "You're going to give her a UTI!" the ol' CNA was just as surprised as me, and proclaimed, "I'm wiping front to back> I said YES BUT YOU ARE NOT FLIPPING THE CLOTH TO A CLEAN SURFACE!

When my mother was dying in a nursing home, her sole complaint when I arrived to spend time with her, was her inflamed, painful privates. She had a terrible rash and it dawned on me that she couldn't have been getting a perineal wash and diaper change, often enough. Sometimes, just wiping them with a wet cloth isn't enough. As a caregiver when a female patient developed a painful rash, a blow dryer would be used on the pre-washed perineum for a few minutes, until the rash was faded enough. Then finished off with a barrier ointment and a clean diaper. If perineal care were done often enough, there wouldn't be a need for "damage-control" efforts, and the patient would be happier. My mother was supposed to be on comfort care, and it only makes logical sense to focus on frequent diaper changes with perineal cleansing. However, no matter how trained a nurse or NA is at perineal care, if they don't change & clean the patient often enough, such neglect can change the patient's "comfort care" into complete agony. It was my mother's only complaint. The worst part was that my mother was a good and caring nurse herself, her entire adult life.That made no difference to the staff. I witnessed the uncaring attitude of the nursing home staff. It was ugly and so inappropriate! The head nurse didn't even hide her apathy & indifference. Of course, an RN or CNA are humans, with the propensity to eventually slack off and neglect patients if they are overworked, underpaid, verbally abused or harassed by bosses ... or just don't care. But you and myself may most likely find ourselves in the position of dying in a hospital or nursing home at some point. How would YOU want to be treated?

It has been a few days already and I have gotten so busy since the day of my exam so late post. I had this exact skill and since I get so nervous I almost forgot to wash the patient backside. However as a women it just felt wrong giving my maniquin perineal care to only one area and no where else lolz. So I hesitated a few seconds and decided if it was wrong it is better to have done extra vs not enough (my brain tends to draw complete blanks if im nervous NO matter how much I know the material). Anyway I know alot of ppl have commented that they passed because of Ms. Patti, Well it is true because I passed by watching and practicing your videos. I found you back in february and actually could not take my test until October 25th, 2024 and obviously I did not study the months in between. I stuffed a shirt and a pair of pants with blankets and I kept a small bucket with all my supplies and used a cosmetic jar to fake the denture part. I kept a privacy blanket, the 4 washcloths you name it , I had it set up for when i would practice. And I just kept going through the motions with my fake patient 2 weeks straight when I was ready for the test and passed!!! Side note I had to reschedule my test date from feb of 2024 since I didn't feel "ready" but if I may be honest since I am a physical learner (initially I never thought of the fake person I made) I wasn't really learning and honestly I wasn't really trying as hard either - or studying as hard as I did for October. Sorry for the long post but I truly did pass thanks to watching your videos Ms.Pattie. Now I will be able to pick up part time while I am in nursing school.

When I took my test my instructor didnt hold mrs mannequin. She was heavy and half way off the bed. My testing was horrible. I even cried before testing started. I really want to thank you for these vids.

Is it ok to ask the instructor to hold up the mannequin while it's on its side if it is giving you problems falling over or are you required not to ask for help like that?

For the exam, the Evaluator will hold the manikin on its side for any skill requiring the "patient" to lie on her side. Hope this helps!

This step is no longer required for testing, so it was removed from the new videos. In fact, the test will not even have disinfectant available for use. Hope this helps!

Patty, what about the side rails, are they not supposed to be up the one the person is rolling towards?

Side rails are considered a restraint and cannot be used without a doctor's order, even for safety. Side rails are not graded on the CNA exam because of this. In addition, they are not present in all settings. It is much more important to learn how to safely turn a patient in bed when siderails are not available for use. You promote safety by scooting the patient toward you, then rolling them into the center of the bed facing away from you. This video demonstrates the proper procedure for safety and is presented according to current Florida testing standards. Best of luck!

You SHOULD talk to your patient, explaining everything you are doing. Not just for the exam, but in clinical life too! Remember that your patient is in an unfamiliar environment, probably scared, possibly alone, surrounded by strangers that inflict pain (IV's, blood work, diagnostic testing, physician assessments, etc.). If you want their cooperation, tell them every step you are doing. Think of yourself in that situation, would you want a complete stranger doing an unfamiliar skill on you in silence? That would be completely awkward, and possibly frightening. Bonus: for the exam, if the evaluator is looking at their checklist but HEARS you do a step, you will get credit for it rather than them missing it because they were looking down at their checklist and didn't see you do it if you are silent. Wouldn’t you have to put up the rail guards when you are sidelining a patient to prevent them from falling off the bed?

No, side rails are not graded for the exam, which means that you have to provide safety as though side rails are not used (even if you use them). By scooting the patient toward you, then rolling them onto their side, they remain in the middle of the bed at all times, which is the safest place for the patient. This is what they are grading on. Siderails are not graded on the exam. You must provide safety without the use of siderails. Asking them to scoot toward you before rolling away from you keeps them in the center of the bed on their side. Thank you!

I was taught in my classes that I could place a towel under the patient as a protective barrier instead of a pad. I'm from Georgia and wanted to know if this was acceptable.

Towels can allow urine to soak through and get the sheets underneath wet. Using a barrier (which is waterproof) will prevent the sheets from becoming wet, so that is the preferred method. Best of luck!

hey patty, I see there's an update version of the skills test, my question is, on the previous videos I noticed you changed the steps to cleaning the basin, you don't use Lysol to spray anymore is that the correct way to clean the basin now?

No, the new standards do not allow for disinfecting the basins. You are simply to rinse, dry and store all patient supplies. Hope this helps!

Yes, that matters. Because whatever you touch with those gloves will contaminate the gloves before you use them to clean. Would you want to be bathed with clean gloves...or dirty? It is important to prepare all of the supplies and apply your gloves immediately before you begin the actual skill. Keep your gloves clean for your patient. Best of luck!

Please any body who can answer this question should please help cuz am really confused. Are you supposed to raised the bed for good body mechanics? If yes when do u put the bed down, before u clean your work area or after u clean your work area? Because the resident safety first.

No soiled washcloths went into the basin - so it only held clean water. You can dump it in the toilet if you wish, but it is not required - it's just water. Hope this helps!

Question . I notice that you don’t lower the bed When you leave to clean the basin . Do you need to lower the bed because you’re leaving the patient unoccupied ? And when you lower the bed can you keep the gloves on ?

I've been struggling with the same dilemma... When to raise, when to lower? I understand Patty doesn't do it because it is not required by Prometic but it would be nice if there was a comment made in regards to it for those of us who do chose to raise and lower. As for the gloves, I've seen videos where they always take off before lowering the bed... but if I do that then do I need to put a new set because I will be cleaning the basin? Not sure. Confused and nervous... my test is in a week. Hope I helped you instead of confusing you more. Wish someone who has already passed can answer or even Patty. I am not sure why comments from years ago show up first and this from you, 2 weeks ago, I had to scroll all the way to the end to find. I kept reading hoping someone had the same question AND there was a congruent reply. Good luck and much blessings to all those who are going through these uncertain moments.

I'm sorry, I'm not entirely sure what you are asking. After replacing the soiled chucks with a clean one, (at 3:40 in the video) I ask Mrs. Jones to roll onto her back please and scoot to the middle of the bed before moving to the other side to repeat the process to remove the soiled chucks. Yes, any time you want the patient to move in a certain way, you should give them directions, or if you will be moving them, you should tell them what you are doing to minimize apprehension and gain their cooperation. Many skills are the same for South Carolina, but the scenario might be different (for instance, ambulate a patient begins and ends at the bed for SC, but in Florida, it is a chair). SC also requires a few skills that we do not demonstrate here (such as putting on PPE, applying TED hose, measuring weight, measuring blood pressure). We have some skills that you will not be tested on. But the ones that we share in common are demonstrated in a similar manner. If in doubt, refer to your Candidate Handbook on Pearson-Vue's website for South Carolina (www.pearsonvue.com/sc/nurseaides/) which lists the steps you are required to perform for all skills. I demonstrated to my instructor what you do regarding peri care he told me that I am wrong using folds to clean between the leg and groin that I should do the pubic and as well clean the meatus clockwise, counter clockwise and again clock wise. I don’t know if prometric will disqualify me applying how you do the skill your way? Please advice me on what prometrics look in skills marking scheme.Thank you

My teacher said in Louisiana prometric wants us to use the mitt ( the folded washcloth technique) watching this I’m pretty sure she was wrong ? Was she right?

Either method is appropriate. The mitt method is an older way of keeping control of the washcloth, but the leaves method is a way to ensure that you are using a different area of the washcloth for cleaning different areas and is preferred to prevent cross contamination. I have a question about the number of basins used in this skill. Our instructor taught us that we should use two basins... one for the soapy wash cloth and the other for the rinsing/cleaning wash cloth. Does it matter if we use one or two basins? Would this be something that we could be docked points for?

Every basin that you get, the patient is charged for. There is no NEED for the patient to have 2 basins, this skill can be done with one, as long as you don't make the water super soapy by adding soap directly to the water. For the exam, you will only have one basin to work with. Hope this helps!
