Integrated Primary Community Mental Health

Traditional divisions between primary and secondary care have made it challenging for patients who are often bounced between services as their mental health needs change.
To better support patients in a blended and integrated model across various levels of acuity and care teams, the NHS has introduced integrated models of primary and community mental health care. This new model creates more seamless mental health support and care, designed to put the needs of individuals and communities at the centre of the way services are organised.
