Free condoms at any public health center in Nicaragua. The birth rate in urban areas is actually not that high. Rural people see children as hands. These people are garbage pickers -- and I'm not sure when the main dump was switched to a different system (paid for by Spain). The government finds that garbage pickers don't appreciate being kept from their work. They do considerable recycling in many areas.
