How do you distinguish between Ocular Myasthenia Gravis (MG), and a motor/muscle problem stemming from a stroke or other issue with the Rostral/Midbrain? Cranial Nerve 3 (CN III) Palsy - PCOMA? Thyroid-Eye-Disease? For example, testing positive for antibodies (AChR, MuSK, etc) would seem like MG for sure, yet the symptoms are exactly the same as it would be for InterNuclear Ophthalmoparesis / Ophthalmoplegia... ptosis and diplopia.. Exotropia / esotropia strabismus ... amblyopia ?

In MG, the ptosis will increase as the day progresses, but in midbrain syndromes it will stay the same along with other symptoms associated with it.
