There are only 2 options of piggyback multifocal IOLs (as far as I'm aware of): Sulcoflex, and Medicontour's Trifocal piggyback AddOn IOL. If we compare the pros and cons, although the piggyback are safer procedures, it may cause positive or negative disphotopsies (taking into account there would be 2 lenses). If a patient is already Yagged, it may be safer to choose a trifocal piggyback, but if the posterior capsule is intact, the best result is with intraocular lens exchange.

Very informative video - many thanks. I have a question, if I may: Is it possible/beneficial to conduct a LASIK surgery for the correction of astigmatism in order to avoid a toric IOL? I can understand that it is one more surgery for those who need an IOL for catarract elimination. Does it make sense though, perhaps for a better clinical outcome overall?

Lasik correction for astigmatism doesn't tend to last for long in most cases. 5 to 10 years after lasik and the cornea will slowly reshape itself into astigmatism. The best and permanent option is a toric IOL. Also, although it's unlikely, the Lasik procedures always have risks of ectasia (even more with astigmatism correction), and if that happens then it tends to be a complicated management afterwards

If a person has astigmatism and has a monofocal IOL, and they want to optimize their far vision without glasses, then LASIK surgery is an option that can be deployed to correct the astigmatism.

When it goes well it’s great. The unknown is how strong the zonules are and how fixed the old IOL is which is difficult to predict preop. If the capsular support is not adequate the patient can be left much worse off or need the services of a vitreoretinal surgeon.

LASIK wasn't an option for me, due to thin corneas, so went with a toric IOL (which has been great so far).

I have EDOF Lenses and cannot drive at night for fear of being blinded by glare and bright lights from oncoming traffic. Question: 1. Does MEDICARE cover IOL exchange in cases where the patient wants standard monofocal lenses to replace EDOF L.? 2. After 5 months of the original surgery, is neuro adaptation to the EDOF L. (glare, etc.) still possible? 3. I have not had YAG. -Initial surgery was done overseas , neither MEDICARE nor any U.S. based insurance was involved. Thank you for your informative videos. I wish I had seen them before my cataract surgery. Looking forward to your response.

You are most inmpressed it seems by the odyssey over panoptix . Im in Eu and some places are reccomending the AT Lisa and the Alcon clareon lenses . Are these better than panoptix ?

What percentage of cataract patients don't like the end results of cataract surgery at all? My sister and several others I know think their vision is worse (hazy etc.) after cataract surgery....and it didn't improve.

Did the patient have any halos, starbursts, light sensitivity? I’m 1 year in from IOL surgery, my eyesight is getting worse. I have Karl Keis Lisa lenses and my eyesight is terrible
