Not the biggest fan of 5-strings myself, but my church pretty much requires those low notes sometimes. Is C# standard tuning on a 4-string a good compromise?

I would go with a 5-string in this context. You'll have an easier time reading the lead sheets, and it will sound more fitting for the context. I would go with C# standard tuning on a 4-string in a band setting, where the guitars are also drop tuned.

How different would this be to the 4-string book, aside from the obvious inclusion of the low B? Any specific content that's only in the five string? What and how much is carried over from the original book?

Great questions! All of the beginner exercises and music from the 4-String book have been adapted to incorporate the low B. The lessons on technique go deeper to address unique aspects of 5-string right-hand muting, the close fretting method, etc. The Moderate to Advanced lessons have been expanded on as well. How To Write a Bass Line, Passing Diminished Chords, and Advanced Picking Techniques are more thorough, especially the videos. For reference, the 4-String Book is 156 pages, and the 5-String Book is 235 pages. The Moderate to Advanced Riffs & Etudes, which make up the largest portion of the book, are all completely different. :)
