The very top portion, above section I, is not clear as to what to be filled in into those 2 blanks. For example: I am filling for a $900 I didn't get it 2023. What goes in those blanks?

That top portion is a little confusing. Sometimes, if you've initiated an inquiry online or via phone, the IRS might ask you to complete this to help their inquiry. In that case, they might send you a copy of this with the top portion already completed. I think you can leave this part blank if you're starting from here. However, there aren't any IRS references that I could find to say this definitively.

What must you fill out in the blanks for in the first sentence at the very top starting with “the information below”? Does it depend on which Stimulus you had/didn’t receive (for the first blank) along with whichever year it was issued (second blank)?

The information at the very top of the form may be pre-filled by an IRS representative if you've been in touch with the IRS in a different manner (calling the toll-free number, online inquiry, etc.). You should be able to focus on Sections I, II, and III. If you're starting with a blank form, I believe you can leave the very top blank.

@@teachmepersonalfinance2169yes I recently spoke to an IRS representative in regards of an expired stimulus check that more than likely gotten lost in the mail when I tried mailing it back (I mailed it a year ago but they said they never received it). So I’m patiently waiting for them to send a copy of form 3911, hopefully there is something that could be done.

@ewing53 I would just stay on top of it until you actually see your Form 3911 come in the mail. Once you receive the IRS version, the top should be prefilled with the information on the top of the form. After you complete it, they should check to make sure that no one cashed the check. If no one did (likely, since it was expired), then the IRS will cancel it and reissue you a new one.

im trying to fill this form out for the 1st stimulus check not received in " tax period" section i would fill out 2020? and " date filed" put the day im filling out this form ?

You should write “EIP1”, “EIP2” or "EIP3" on the top of the form (in this case, EIP1). Enter 2020 as the tax period, and then enter the date that you filed the tax return to claim the check.

Would I need to complete a separate form 3911 for both the federal tax and the state tax refunds?

IRS Form 3911 is only for federal tax refunds. You would have to check with your state's tax office to see what their procedures are about tax refunds.

Yes. If you do this, and your check has not been cashed, then the IRS should issue you a replacement check after they cancel the original.

I received the first stimulus payment never cashed it and it expired. IRS lady on the phone told me to fill out this form. For "type of refund requested" do i put my bank account i want the money sent to or do i select check because thats what i received it as? Also for tax period i put 2020 but confused how what i put in date filed? Last question do i select the "lost" box in my case. thank you for your time!

For type of refund, you should select the way you want to receive the money. For date filed, you should look up the date that you filed your 2020 tax return. You can say that your check was lost or destroyed.

​@@KeegNWZ I'm not sure. I would be inclined to call the IRS back, and discuss this over the phone with them in more detail, so they can give you specific guidance on what to enter here. You can also use this as an opportunity to see if there are any other fields that you might treat differently to replace your stimulus check. There is another page on the IRS website with guidance on replacing a stimulus check: -Call us at 800-829-0115 to request a replacement check. -If you have the expired check, please destroy it. -When you receive the new check, remember to cash it.

Can you use this form if you have some type of hold on your refund that's hindering it from being reissued again? Or will the hold need to be resolved first to have your refund released? Also, if originally you requested to have your refund issued in direct deposit in an account, but now you want it direct deposited into a whole other account in which you will provide to them on this form; will they honor it? Also, is it guaranteed that this form will indeed send out another refund if applicable? And lastly, what's the standard time frame for the refund to be reissued after this form has been submitted?

If there is some sort of hold, then you might need to resolve that issue first. Most common instances would be the Treasury Offset Program, or prior tax debts. The Treasury Offset Program is used to divert tax refunds to certain federally recognized debts, such as state taxes, child support, delinquent student loans, and other specific situations. If you're married, and your refund was used to offset your spouses back debt (that you're not personally responsible for), you may be able to file IRS Form 8379, Injured Spouse Allocation, to get your share of the tax return back (but not the entire return). www.teachmepersonalfinance.com/irs-form-8379-instructions/ You can request that your direct deposit go into a different account than the one stated on your tax return. However, there is no guarantee that the IRS will honor your request...depends on the circumstances. There is no guarantee that the IRS will send out another refund. There is a process that the IRS follows, to determine how the payment was sent, and whether or not the refund actually ended up in the taxpayer's account. For example, if you were issued a check, the IRS will see if it had been deposited. If it has, your options are limited. If not, the IRS might cancel the outstanding check and issue a new one. Generally, the IRS can take between 6 & 8 weeks to process this form and issue a refund. Hi Sir , i have question for this State field , i am from Serbia - Europe , and as i see i can only type 2 letters there , so should i put there RS - thats only two letters i can put for my country , or to put in City field - my city and country both ? thanks for answer

I would probably contact the IRS for guidance on this. However, my first inclination is that the IRS (and U.S. federal government) use a standard two-letter abbreviation for countries. In this case, RS means Russia, so I would not do that. For Serbia, the abbreviation is RI www.irs.gov/e-file-providers/foreign-country-code-listing-for-modernized-e-file-mef But contact the IRS to make certain.

I deposited a refund and it went through but the next day it was removed, turns out it had expired(?) the check was just over a year old. Is this form the only way I can get a replacement? And what do I put for section 8? Because I did receive it but I no longer have the check and it's no longer valid

In this case, I would defer to the IRS instructions on the form itself: If you are in possession of a check which was not cashed within one year of the issue date as the law requires, it can no longer be cashed, contact the service for instructions on how to return your check. Even if you're not in possession (because the bank took your deposit), I would contact the IRS about getting a new check re-issued.

I mailed a Form 3911 almost 5 months ago and have gotten no contact or response from the IRS. Trying to call them to inquire has been frustrating as none of the options take me to a human. How can I get help to see if they even received it and what the status is?

It seems that a lot of taxpayers are in a similar position, specifically because staffing levels at the IRS are pretty low. However, the IRS prioritizes taxpayers who indicate that their missing refund imposes a financial hardship. If possible, you may consider setting up an appointment with a local IRS service center. Another option might be to contact the Taxpayer Advocate Service to establish a case to help expedite your refund. You can find a local taxpayer advocate office using this locator tool: www.taxpayeradvocate.irs.gov/contact-us/#FindlocalTAS However, the taxpayer advocate is also backlogged. You might find some success in filing IRS Form 911, Request for Taxpayer Assistance: www.teachmepersonalfinance.com/irs-form-911-instructions/ Finally, you might check the 'Where's My Refund?' tool on the IRS website. It is slightly possible that the IRS is processing your refund, and has updated the status here.

What if I downloaded this file and don’t have the information for the first line, where it states “reply for inquiry on..” etc.? Do I just leave it blank? Can I send it online or does it have to be through mail?

Unfortunately, you cannot send this form online. You can fax it or mail it to the IRS Refund Inquiry Unit for your state. You can find this information in our article: www.teachmepersonalfinance.com/irs-form-3911-instructions/ If you downloaded the file, and are not completing the form in response to IRS correspondence, you may enter 'N/A' in the inquiry fields, or leave them blank.

You may be able to use an envelope with a stamp to file Form 3911, in accordance with postal regulations. However, to safeguard your personal information (like your SSN), I might recommend a service with a return receipt confirmation, such as certified mail or FedEx/UPS.

What if my check was stolen and potentially cashed by someone else. Do i use the same form or some different form?

You would use this form. If the IRS traces your check and determines that it was cashed, then they would refer you to the Treasury Department’s Department of Fiscal Service. They would then initiate procedures to locate the person who cashed your check. After their investigation, they would then determine whether to re-issue the check to you, based upon the results.

If you can still use the old account, you should consider entering that account information, because it will help the IRS to be able to trace any payments that they processed. If there is no way that you can use the old account information, then you may want to first use the 'Where's My Refund' tool to see whether the IRS paid your refund in the first place: www.irs.gov/wheres-my-refund

I made a mistake and filled this form out because I thought I didn't put the correct direct deposit info , but I did . It did deposit in my account . Would they take the money back or they let me know that it was deposited in the correct account .

If you're worried that the IRS will blindly send you a second check, I don't think that would happen. Most likely, the IRS will trace your payment to your account, then send you a notification that their records show evidence of this deposit. Unless you choose to push back, this probably would be the end of the inquiry.

As long as the check wasn't cashed, then you should be able to ask for direct deposit to replace the check. If the check was cashed, then you might need to file a claim with the Treasury Department before you can get your refund.

@@eman-id8rw Usually, the IRS will attempt to call the listed phone number. If they cannot reach you, then you’d probably receive an IRS notice with written instructions if they need you to take action.

I would probably talk to the bank first, to determine why your refund was rejected. Then I would use that information to determine your next steps with the IRS.
