I greatly appreciate this channel. As a clinician I find that a simple, fundamental isometric protocol generally works really well to treat tendon related pain, regardless of the location. Really high quality content, keep up the good work!

i was wondering the whole time why you would want to decrease pain so that you can then overload an already pathological tendon! To help do rehab it makes sense, but to compete/exercise, it seems foolish! Glad you got to this at the end! Thanks for the great videos and sharing of knowledge.

It's an option for high level athletes that cannot be rehabbed in-season. If they don't get progressively worse, then there is no need to pull them from competition, but isometrics can be a way for them to deal with the pain

Across all studies it seems that the average effect is not there. Might be the same with stretching (works for some, not for others) but in doubt would always opt for load as it makes people stronger as well

Hello physiotutors, can you guys please do a video regarding the use of therapeutic ultrasound for soft tissue injuries/tendinopathies? I am seeing a lot of conflicting information regarding this modality. Are there any cases at all where ultrasound use is appropriate? From what I've been reading the mechanism boils down to heat = increased blood flow = faster healing but have never seen evidence for this claim

We don't have any knowledge of evidence of efficacy for ultrasound as a therapeutic intervention. A progressive loading program is the absolute basis. Every other modality (for example shockwave is optional).

google for "achilles tendinopathy runningwritings" and read it. Basically, rest from aggravating activity for a week, then start eccentric calf raises.

@@stacer1962 I feel that this is a position of privilege. The evidence based first line of any Achilles tendonitis patient is to start with eccentric calf raises. This is very clear. In what way would a patient be so unique, that you alter the treatment, and what treatment would you apply instead, which runs counter to all EBM guidelines, my dear med student?

Hello sir, very nice video. i need your help please help me. when i am doing leg swing, straight leg raise stretch and Deadlifts i feel pain full hamstring and in some groin stretch i also feel some pain but sir pain is not sharp like in strain. why? it is happening or how can i cure. please help me.
