have you considered surgery I had that it did help but I can no longer run jump or climb like a tree or anything I never could do much of that when I was younger but after the surgery that basically put a stop to it or think about maybe putting him in a wheelchair not permanently just temporarily for a long walks and stuff if he tires out like I doto be honest I don't think the surgery really helped me I'm in constant pain with my feet but it might help you
