Hi Dr., may you help with this problem? Thanks. Medical intern ordered Morphine Sulfate IV 5 mg/hr for a patient. Only Morphine Sulfate concentrate 25 mg/ml is available. What would be the infusion in (ml/hr), if 5 ml of the Morphine Sulfate is added to 245 ml normal saline? Thank you.

Great video! Thank you! I have a problem. Can you help me? 45 year old female patient (70kg) is on Vancomycin 500mg infusion(1hour duration) , every 8hours. Cpmax is 25mg/ L and Cpmin is 14mg/L. If you know that Creatinine clearence is 30ml/min, can you determine the Vd and Ke? Also can you suggest new regime where Cpmax is 25mg/L and Cpmin is 7mg/L?

This helped me a lot! But I got this answer wrong on a quiz last week. And I’ve been trying to figure it out ever since, can you explain it, Order: amikacin sulfate 7.5mg/kg IVPB q8h in 200 mL D5W to infuse in 30 min. The vial reads 500 mg/2mL. Calculate the flow rate of this aminoglycoxide antibiotic drug in mL/h for a patient whose weight is 144 lbs.

Thank you Lorena for watching and for the comment. First convert the 30 minutes to hours using the conversion factor (1 h = 60 minutes). Hence, (30 minutes/60 minutes)x1h = 0.5 h. Now, flowrate = (volume/time). This implies flowrate = (200 mL/0.5h) = 400 mL/h.

200 ml/30mins x60mins = 400 ml/hr... that's all you need to do.
