I was confused by your references to the number being small. My trick is if the solutions are 0.33, 0.225, 0.5 or below 0.9% NS then the solutions are hypOtonic. The Zeros of the solution remind me of the O in hypOtonic. For hypertonic everything above 0.9% is hypertonic (3%, 5% etc.) especially the large numbers. Hope this helps someone

Saunders comprehensive Reivew for NCLEX-RN has D5W as isotonic. I didn't understand your rational for hypertonic having small percentages of NS. The percentages for NS for hypertonic are actually BIG compared to Iso and Hypo. Hypo (0.33%, 0.225%, 0.45% NS), Iso (0.90% NS), Hyper (3%, 5% NS). I like your memory tricks in the videos!

This is true, D5W starts out isotonic but then the body metabolizes the dextrose and the result is a "hippo-tonic" solution.

Can you explain who would get which fluids. Like what would a CHF patient get? and why? What would a dehydrated patient or a burn patient get?

Priscilla Ruiz I don’t know if you need the answer 3 years later...but for anyone reading this. A CHF patient probably wouldn’t have fluids ordered, if they did it would be administered slowly with constant checking for overload, it would also depend on the indication for fluids (surgery, etc) usually the fluid rate for CHF is less than 100ml/hr, closer to the range of 50-65ml/hr A burn patient would receive LR or NS (probably LR since it’s a crystal colloid, contains electrolytes and will restore volume). The Parkland formula is the calculation for volume repletion.

You are essentially correct- D5W is actually an isotonic solution while in the bag but is considered a HYPOTONIC solution once it's absorbed into the body. If we are talking about 5% dextrose in NS...then that's considered a Hypertonic IV solution. Hope this clears some things up!

5% dextrose in water or D5W is isotonic however, 5% dextrose in 1/2 NS and .9 NS is hypertonic, as well as D10W. don't get confused!

why does he keep saying .45% is bigger than 3 %? its significantly smaller... its less than 1

Marcela Gomez I don't think he's saying its smaller because it's a hypotonic solution making it smaller anyways....I think it's just a way of memorizing it, but I did notice that he's says all the dextrose solutions are hypertonic when D5W is NOT hypertonic....its an isotonic so hopefully everyone here knows that :)

he is making it more confusing... think hypertonic uses big numbers as in whole numbers... hypo (think hypo as in low) you get low numbers the 0.45, 0.225 or 0.33.. he def is making it more confusing than it needs to be

Does anyone have an online source anywhere to learn how we can choose which hyper/hypo/isotonic solution for which clients' condition? Like I am still weak in knowing what to choose when a person is hypovolemic/hypervol/dehydration

My med-surg course also considers D5W as isotonic solution; Linares is right though; once the dextrose enters the body, it metabolizes so all that's left is free water. I guess it's common that some nursing schools consider it to be isotonic.

D5W is not hypertonic. This is very misleading. D51/2 and D5NS are hypertonic, but D5W is hypotonic in the body.

D5W is not a hypertonic solution. It is Isotonic in the bag but, once metabolized in the body it becomes hypotonic.

I'm so confused.. our teacher said that anything greater than 0.9 is hypertonic so wouldn't that mean that hypertonic solutions are bigger than hypo solutions.. please clarify, i have a test coming up!

tee h 3% and 5% is greater the 0.9% making it a hyper and anything below that 0.9% is a hypo...wish i would have seen ur question earlier so that I could help with ur exam!! Hope u aced it :)

+Brandy Morse Thank you so much for replying. I was confused on the percentages. Eventually, I converted and wrote everything down in percentages and it made more sense to me. I am almost done with my final med-surg class. I will be graduating in May :) Thanks again for being so kind.

Yeah what he says it just totally confusing. Hypertonic solutions are NOT smaller numbers than hypotonic solutions. However...what he means is that if you ignore all the zeros then hypotonic solutions are bigger. e.g. 0.33% saline if you ignore the "0." you get 33. Which is a big number. Although it sounds like a dumb and confusing way to look at it - there is one advantage. It reminds you that hypotonic fills cells and hypertonic shrinks. Though I would just see the numbers AS THEY ARE hypo = tiny and hyper=bigger and just remember hypo=Hippo. anyway that's just me - long post. And oh yeah dextrose 5% is isotonic (in the bag) and quickly becomes hypotonic in the body. Don't listen to him saying it's hypertonic - that's totally wrong.

Maybe he said it mistakenly... from what I understand, D5W is isotonic because it has the same concentration as the body fluids. But as it enters the body and gets metabolized by the body, the D5 part which is dextrose or glucose and it loses it. So the concentration of D5W is reduced due to the glucose being metabolize, so it is now a low concentration compared to the body fluid or also known as hypotonic. That's my understanding, I could be wrong.

Comes down to osmolarity, dissolved substances in the fluid or % of solute compared to that of blood. NS at 9% is isotonic which is close to blood osmolarity, so no fluid is exchanged. When the solution is less than NS 9% (0.9) in terms of decimals such as 1/2 NS (0.5 NS), 1/3 (0.33 NS), 1/4 NS (0.25) means that the solution given has less osmolarity than blood or 9% NS, it becomes hypotonic. Cell takes in or loses water r/t hydrostatic pressure, hypo=low hydrostatic pressure, hyper=high..etc

I'm pretty sure D5W is hypotonic...I'm taking a kaplan review course for the NCLEX and one of the answers said it was incorrect to give D5W because it was hypotonic. Now I'm confused.

This was very helpful. However, what I am stuck on prior to taking the NCLEX, are the which fluids do you use for specific conditions. He mentioned that Isotonic are used for hypovolemic clients. What else? What conditions warrant Hypotonic fluids and which warrant Hypertonic fluids? Help!

I'm only in my 2nd semester of nursing school but usually after a blood/electrolyte test are you able to figure out which solution to admin. But in emergencies, i suppose you could look at the patient's vitals along with objective/subjective data to determine which to admin. I might be wrong but its just a thought.
