You should try Riftmaker>Titanic>Visage>Sterak>Frozen Heart WW. thats what i was playing some time ago and it scaled incredibly into late, now probably Ravenous is Better than Titanic but the rest can stay the same

You could also try the same buuild but instead with Riftmaker as replacement for Jaksho and switch to Ravenous Hydra, you get more squishy, but the reward is overall better health, extra AD thanks to Ravenous, and most importantly a 18% Total Lifesteal instead of the mere %10

It works like a charm for jungle. Makes you ridiculously tanky as well which scales so well into the late game you'd be surprised how often you live where you normally die and that usually makes the difference between the enemy team over focusing on you or not.
