Because aviation learns from lessons of the past. Look up Air Canada Flight 797 & British Airtours Flight 328. Fire safety regulations and certifications today largely came out of those accidents. As a result, we got the successes of Air France Flight 358, China Airlines Flight 120 and this accident at Haneda where everyone got off the burning planes alive. It wasn't just luck, nor was it the discipline of the Japanese people or the 7 star safety rating of JAL. It was due to hard lessons learned from disasters past that saved their lives.

That "wow" is due to Japanese discipline. Look at some older emergency exits from aircraft in Russia, big men grabbing their suitcases and pushing women and children out of their way. It is a cultural thing!

Even more impressive because the passengers only could use the exits at the front of the plane. It was designed to use 8 exits in evacuating everyone in 90 seconds. It's amazing everyone got off safely using only 2 exits. The folks in back had to walk all the way to the front, while the cabin filled with smoke and fumes, and the fire spread.

It was not only the crew who saved the lives of the Airbus crew. Even their behaviour itself. Don't panic before and during the evacuation. Social behaviour is different from that in Europe or elsewhere in the world.

If their time is not over and the building fell on them, he can still keep them alive, why? because he can. All she was saying is praying for them to be alive, it's natural for humans to want each other alive as opposed to dead, however at the end the day, it's God's decision. I am open to any other logic you may have but I will respect if you don't believe in God.

The tower records everything as well btw, not just the planes. The air crews are supposed to read back all critical instructions. So if the government's claim of pilot error is correct there should be an instruction from the tower to hold short and an acknowledgement of that restriction (by a read back) from the crew on every recording device.

yes and sometimes they even have different terminologies and phrases they use to traffic control. this is true for US Civilian and military airports. the ATC was telling him to "taxi to holding point C5 (charlie 5)" which is a valid command under ICAO, this means hold short at Charlie 5. he went on the runway anyway.

It shows that Japanese people follow protocol, are organized and law abiding, and care for each other's safety. What a culture! Imagine that in America - most passengers would stomp on others and pandemonium would ensue. No manners at all. Thank you, Japan for such an excellent aviation safety record and protocols. You forget to mention that in the US, the Cabin Crew of Size from our diversity hiring would be obstructing evacuation. I am a frequent flyer and it still amazes me that US airlines hire cabin crew that are so overweight that they can barely walk through the aisle during service.... or that senior/advanced age crew with shaky hands and looking frail.

In the event of an emergency, in the U.S., passengers would get killed by being trampled, pushed, and shoved while trying to escape. And corporate culture in the U.S. and Japan are vastly different. In Japan, they hire for merit and excellence, words which are forbidden and considered racist and exclusionary in the U.S. Merit means who is the best person for the job. Merit doesn't mean everyone old, fat, young, skinny, and of every race color or religion gets represented in hiring. The 2 goals do not coincide. Here in America, the focus of corporations is on hiring for representation: diversity, equity, and inclusion. You don't have to be THE BEST for ANY job, just mediocre enough to be hired. And in the U.S., it doesn't matter if people die during a plane emergency because, well, quite frankly, the American public is cynical and the expectation of the American public is that people are GOING to die in an emergency. So American corporations don't hire based on merit because there's no benefit to the company for doing that. But if you don't have a high enough percentage of your workforce that's female, elderly, minority, or obese (which can help a company meet its handicap hiring targets), then you have the feds AND the media on your case for racism and discrimination. You will also make the news for having a workforce that doesn't reflect America. You will also get sued for having a nondiverse workforce and lose money. You can have an all-white, all youthful crew that saves everyone's lives in a crash but no one will be thanking them for saving everyone's life. Instead Americans will complain how racist and unrepresentative the flight crew is, day in and day out, when there IS NO emergency. In Japan, people don't want to have old, fat flight attendants. They won't hire any attendant above 45, they won't hire if you're even slightly overweight or unfit, let alone obese. They care about surviving a plane crash in the event of an emergency. They want flight attendants who can lift 100 pounds, who can push open an emergency door. We can't have physical requirement tests in the US because it violates the Americans with Disabilities Act of 1990. I work in HR. The priorities of the American flying public and the Japanese flying public are different so airlines cater to the needs, wants, and the situations of each country.

If it were recorded in the audios, best believe a government would run with ANY reason to blame the tower and NOT their own military. If you haven't noticed, the tower didn't see the plane on the runway, and it can't be seen in the video until it's struck. That is a big red flag. The tower doesn't just look on a screen and talk to pilots. They look at physical runways as well. Clearly NO ONE saw it there. WHY? Where were its lights?

The pilot made a mistake and misunderstood the instruction to taxi to holding. Instead he went on the runway and readied for takeoff, even now he insists he was given permission to taxi on the runway. Since he was facing takeoff direction and the airbus was landing, it came directly behind him, nose up in the dark. The coast guard plane wasn't equipped with a modern ADS-B transponder. Neither of the pilots or controller would have seen each other visually, or via radar.

It's curious that communication between such important control towers and pilots is done only verbally. Currently, what is being reported in Japan is the suspicion that there was a misunderstanding in the meaning between 'Please wait before entering the runway' and 'Please enter and wait on the runway' in Japanese.

They did release ATCs instructions to both planes, and Coast guard was given instructions to hold just short of the runway and JAL given clearance to land. They haven't released pilot responses yet, but I wonder if the Coast guard plane's captain misunderstood the ATCs instructions? I thought at first maybe they thought they'd stopped where they were supposed to and just didn't realize they were in the active runway, but looking at the airport layout there's nowhere for them to go EXCEPT the runway, so all I can think now is a misunderstanding of instructions or misattributing instructions to themselves.

Sure the CG plane was in the wrong spot but i’m sure the investigation will cover why ATC didn’t see the plane was in the wrong spot before the collision. This sounds like a repeat of the LAX collision from 1991. There’s never a single point of failure (or blame) when an event like this happens.

If a government is placing the blame for a catastrophe squarely on its own military, believe them. Whatever that pilot believes, he clearly did NOT have clearance from the tower. If the government had a single shred of reason to believe they could blame it on the tower, they would. They'd find a flunky to blame it on if they thought they could get away with it. What I can't understand is why didn't anyone in the tower SEE the CG plane on the runway before clearing the commercial plane to land? I find it interesting that it can't be seen in the video until it's hit. Where were its lights? Is that why no one knew it was there?

Yeah, it was dark little thing sitting on the tarmac. Looks like its lights are not bright enough and the surrounding area is not well lit. Or it just blended into the surrounding lights. Very hard to tell. I bet the A350 pilots could not see it well either in the darkness, though this is probably speculation on my part. I bet had they seen it from a distance they would have done a fly around, rather than land. Darkness also played a part I suppose.

The tower is a long way from the end of that runway on the other side of the airport. It was at night and the lights of the plane would be hard to distinguish between the surrounding runway and taxiway lights. Same applies for the view from the A350. The white tail light of the CG plane would be right on the centerline of the runway and blend in with the white runway centerline lights. The wingtip lights are easily lost in the runway edge lights. I’m curious though, why the A350 crew didn’t see the CG aircraft on their TCAS display (which shows other transponder equipped aircraft on their navigation display). That would have indicated that there was still an airplane on the runway. I understand that the CG plane may not have been equipped with ADS-B out - which would also have helped make them more visible to the A350 if their traffic display was selected on.

Hopefully this will push for wider adaption of RWSL (Runway Status Lights) - as is now operational at about two dozen US airports. These lights embedded into the runway entry and centerline, give light signals to the pilot if they are cleared to enter the runway or takeoff. Thus providing some protection against misunderstood radio communications (which looks like it may play a part in this accident).

@@MeerkatADV Thanks I just saw that. Also heard that the system has been inop for the last week or so. However,looking at the NOTAMs that’s not clear. However that system works automatically to prevent aircraft from taxiing or taking off on a on a runway that is occupied. In this case it was not (other than by the CG aircraft), so it may well be a moot point.

@@MeerkatADV The RWSL system doesn’t actually have green lights (for GO). But the taxiway lights are green and they are shut off when the red lights come on. When the runway is clear of other downfield traffic the red lights automatically turn off (and the taxiway lights can come back on) that is NOT a clearance to enter the runway. I wonder if that pilot realizes that ATC audio is recorded and that particular audio is broadcast globally online. He can deny it until the cows come home, unless he has an encrypted frequency where he got conflicting orders, which is doubtful, he's just digging himself into a deeper hole and increasing the chances of him going to prison instead of just being shamed.

The fact that the pilot claims he was given permission to runway clearly shows this was the case. The controller and airbus follow the routine constantly whereas the Coast Guard are visiting and wouldn't be familiar with the layout of the airport and usual comms.

Other reports tonight say that records show that red warning lights designed to stop taxiing planes from entering the runway were out of service at the time of the deadly collision, but it's never one thing. It's usually a series of mistakes or errors.🤔🤔

If the captain of the DHC-8 was in fact cleared for takeoff then the responsibility lies on the ATC but the PIC of the DHC-8 should have also exercised proper discretion before entering an active runway.

Listen to ATC recordings and you will hear them direct the Coast Guard plane to its holding location at runway 34R, but not specify to hold short of the runway. The pilot assumed he was clear to enter the runway and await clearance to take off, when he was actually supposed to hold short of the runway until cleared to enter and take off. This was the fault of ATC, for not being clear with instructions AND the pilot for not repeating the message back for clarification.

When directed to a holding location, it would always be for holding purposes. Else the ATC would just tell them to taxi straight to the runway and "LINE UP FOR TAKE-OFF". The transcript have been released and the Coast Guard pilots did the correct "READ BACK", which they are SUPPOSED to anyway.

Those passengers were just lucky enough to be in arriving flight instead departing one. If it was other way around then it could have been worse...much worse due to fully loaded fuel. This case it was coast guard who were heavily fuels that's why they burst into fireball straight away.

when a firetruck in an emergency with siren blaring sped pass a red light, everyone have to give way. But when it tries that over a train crossing, it get smashed to bits by the train. the Emergency crew says " we have priority!. train say:" we can't stop" Same thing here. the coast guard pilot, it is an emergency i HAVE priority. A350 i cannot stop.

Oh, so tragic. When natural disasters strike, things get crazy out of protocol. The flight attendants did exceptional, however at getting the passengers out to safety. Looking at the remnents, there wasnt anytime to loose.

When a pilot has landing clearance in today’s airport safety age, it’s so easy to not even bother looking and focus down the runway because you are 100% convinced that the tower knows what they’re doing.

A Tragedy. I am not a Pilot but I know that an aircraft, when landing, has a 'nose-up angle of attack which obscures an aircraft 'holding short' to allow the cleared aircraft to land. I am puzzled why, in this age of miniaturisation of dashcams and the like, why there is not a camera beneath a landing aircraft (screen in the cockpit) to show an aircraft encroaching the runway. It may not be the solution in every case but at least it would give the Pilot and Co-pilot of the aircraft 'on approach' enough time to execute an emergency 'go-around'.

Make a suggestion to your powers that be, pilot union etc so they can pass it on to plane manufacturers. Sounds like a brilliant idea. Another camera capturing the back view on the tail for viewing landing planes from the sky would also be awesome. These two could be part of the landing and/or take off procedure if such a thing exists. However, in heavy rain, they might still not be enough.

All planes land with that nose-up angle. The A350 even has a very slopped nose cone design compared to the B777. B787 is better than B777 in this regard. The final descent just before touchdown would have the pilots not have view on the ground, this is pretty common. Even if the pilot can see, it's pretty late at that point. His speed is too slow to conduct a go-around but too fast to slow down before hitting the Dash-8. There is not much the A350 can do at that point. Maybe, maybe if A350 rev up the engine and sped up, it could slice through the Dash-8. The moment the Dash-8 entered the active runway, it's pretty late to recover.

A350 is/was my favorite as I ride it often. This accident is making me rethink. Multiple interviews indicate multiple systems failed onboard the A350. Intercom was down, electronic door control was down, some lighting elements were down. The crew had to resort to manual everything. In terms of impact, this isn't as serious with the left engine clipped by the Dash-8 or clipped the Dash-8. The fact the plane is already on the ground and this type of impact can bring down multiple systems is very concerning. Plus, the composite material used on the A350 seems to be the perfect fuel for fire. It's rare to see a plane, nearly empty of fuel, could burn into a crisp like that.

@@_w_w_ Your "facts" are incorrect. Your fear is irrational. The fuel came from the Dash-8 and the composite fuselage actually burned slower than an all aluminum one. No one died. Two planes colliding and not one fatality aboard the A350 is a huge accomplishment for Airbus. It's evident that this is a truly safe airplane.

Yeah! What the heck were they doing on the runway?? Anybody looking out the window of the Coast Guard plane? What frequency were they on? Ground or tower control? They didn't notice the bright landing lights headed their way? Unfortunately they had their head up their butts and paid a dear price for that.

They were told to hold at C5 but went straight into the center of the runway right before TDZ(Touch down zone)

Was thinking about this today RE: planes moving onto the runaway to take off, I don't much about how it all works, but surely when an aircraft is about to move onto the runway do they not physically look for any possible aircraft coming into land as a last chance safety procedure, like on a motorcycle you have the "life save" look over your shoulder, or do they rely 100% on traffic controllers without looking themselves? Aircraft have landing lights on as they come into land.

You are like me. However, most airports where I live don't hold on the runway. I know because i always watch when flying how planes move along until they are on the runway. There is a holding place on the taxiway near the runway where the leaving plane waits for the landing plane, then after the arriving place lands the plane waiting to take off moves from the holding point onto the runway, ready to fire its engines to max for takeoff. I always marvel at the ingenuity of that system.....until this accident. Then I realised that I know nothing at all. Perhaps the airports where I live are not as busy, therefore have the luxury of using less space on the runway than for taking off. Strange system though. I think it should be changed, else more accidents will happen.

The holding place is next to the runway. The planes don't go on the runway to get to the holding point, there is a separate road system at the side for the planes to move around the airport. The coast guard was told to move to holding at C5 but he didn't stop there, he kept going and went on the runway. Pilot error.

But I heard that the Pilot from coastguard said he was given clearance to take off? so he knew he was going onto the runway, what my first comment was is do pilots not do a visual check for possible aircraft coming in to land for a final safety check (in case there has been an air traffic control error) ?? It would be like being in a car and relying on your passenger to tell you all clear without doing a final check yourself!

@@Waynzo-i3w The pilot misunderstood the instruction to taxi to holding. He thought he was given permission to go to the runway. He was on the runway waiting to takeoff. The airbus landed behind him with nose up in the dark. Neither pilot would have seen each other. Also at night time you can't see anything in the air and there are several airplanes circling waiting so you can't tell what plane is coming in.

Real SIMPLE solution. Two parts: 1) Put RED and GREEN lights at the runway taxiway(s) entrance to the runway. At the Hold Short line, put a normal signal light. 2) Insist and monitor that ATC Controllers speak CLEARLY and enunciate every word properly. Try to understand what the ATC Controllers said to the Coast Guard right before the accident. Good luck trying to figure out what was said.

Did both ATC instructions state "stop short of C1" or did the second say "stop at C1"? It may explain the runway incursion. Of course, that pilot is supposed to study and remember the relevant parts of the airport layout, and know not to stop on an active runway. Perhaps he forgot or lost his bearings.

@@TrFusionYou are so wrong and confident it's funny. C1 was for DAL276 a Delta flight, not JA722A which is the Coast Guard's plane. JA722A was told to hold at C5 which they did, but they proceeded to the runway for some reason.

In the US pilots must reply Hold Short, of the runway, and its corresponding number in this case 34R, to make sure they are not on a runway that has landing traffic on it. The transcript I heard does not have this reply from the Cost Guard aircraft, They say hold point which may have been misunderstood as "position and wait" meaning enter the runway and await takeoff clearance. Sadly when non-native English speakers hear something they understand something else, Which is also true for native speakers. English is a non definitive language that causes many problems.

Taxi instructions to a hold point are as good as a hold short. Acknowledging the hold point and blowing onto the runway places the blame squarely on the pilot
