Why do some boxes of this headset have a silver label on the left that says JBL Quantum Surround and other boxes don't?

It's not wireless so you need to keep it plugged in on USB

Are you sure you plugged the right end of the 3.5mm jack? Also, try working the Game/Chat balance wheel to make sure the audio output is on the right channel.
