Nice to see the callouts on some of the open questions in FAO/ketogenic metabolism. Curious that he went back to Owen and Reichard's work but didn't touch on acetone, up to 70 or 80 percent of which is utilized throughout the body, not merely exhaled as so often taught. The question of compartmentalized NAD metabolism (AcAc from liver, conversion to BOHB in muscle) is fascinating and has been nagging ever since I first heard him talk about it after the FASTER study. And what about that 8-fold increase in Hmgcs2? Muscle and astrocytes pose a similar paradox: expression of both the rate limiting enzyme of ketogenesis (Hmgcs2) and the rate limiting enzyme for ketolysis (SCOT). Synthesizing ketones from acetyl-CoA and then turning around and breaking the ketones back down to acetyl-CoA? The export of BOHB would make some sense of this if the mechanism revolves around NAD homeostasis. Verrry interesting. Thanks for sharing the video. Ignore the complainers - yes there are ads, but yes also this content was shared FREE. ❤

Thank you for this information. Very Low Carb and/or Nutritional Ketosis Diets (NKD) have been used since at least 1922 (and earlier) to control Type 2 diabetes, reduce insulin requirements for Type 1 diabetics, and reduce epileptic seizures. More recently, NKDs have been used to prevent, delay onset, and/or mitigate other pathologies. Presumably, some diabetics and/or epileptics have been in or close to nutritional ketosis for several years (10, 20, or more). Given this 100-year history, do you know of any longitudinal, multi-year population observational studies (or clinical studies) of diabetic and/or epileptic (or other) groups that document the positive and/or negative long-term health impacts of NKDs for 5, 10, or 15+ years? The longest study I have found so far is 24-weeks, not long enough to identify some situations. Thank you.

If you are long term fat adapted eg 3 years would doing 4 or 5 hours cardio in the middle of a 48-hour weekly fast for a number of weeks upregulate glycogen re synthesis via gluconeogenesis or would it down regulate glycogen use during this exercise?

I want to add my voice to the KETO community...before I die! Look, I KNOW it is JUST ANOTHER VOICE. N = 1. But we can all add up to something significant (to counterweight the bad science/vested interests out there). We should ALL subscribe to folk like Jeff Volek (it's free to subscribe, after all). Google takes note of the numbers. Perhaps governments will too?
