Theory as to why Perhaps Pirate Jo has a skull on his bandana: He got a head injury when he fell off his ship, and thus when Sir Piggles rescued him, I don't think either of them had much in the way of medical supplies, especially bandages. As a result, Sir Piggles had to improvise medical supplies and used one of Skeletron's banners as a makeshift bandage to clean up the blood and apply pressure to the wound. Pirate Jo hasn't taken it off/traded it out because it helps keep his scalp from getting sunburnt, he doesn't know how to make his own bandana(s)/bandages, or something else.
