The Canadian version has custom labels as do all the Canadian polydor 45s and singles from John and Yoko. Also, the insert you have there, is taped to the back cover on the Canadian issues.

I inherited a US original pressing in my father's collection, complete with insert as shown here. I say original pressing, but I don't think it was ever reissued, so every copy out there is probably a "first edition" for whichever region.
