Well, there are two things to consider: - Stands are invisible to normal people, so what everyone else would be seeing (assuming they would even pay attention, since they are all minding their own business and Narancia is keeping a low profile... blood allowing!) is just a guy with a knife and not the tongue shaped Stand on its tip. - Narancia is correctly assuming that only his enemies would actively monitor every move he makes, so they would be the only ones in the crowd that would be shocked by seeing his tongue cut and replaced by Giorno's ladybug and consequently altering their breathing.

I think Narancia just was waiting for someone to panic big time. An ordinary person would be shocked but it’s still just some random guy. For Tiziano, their only defense left against the enemy with a flying, machine-gun blender failed.

Okay funny and true, but apparently it doesn’t actually work like that, apparently araki explains that yes, the characters are drawn with eccentric clothing and hairstyles, but in reality they look normal to everyone, but stand users see them differently then normal people, so to a normal dude they look duller and more normal, but being a stand user makes them stand out specifically to them, so it’s like when you have a invisible ink pen, normally you can’t see it, but all the stand users see in black light so they can tell it’s there
