Giorno is unkillable, but not immortal. He CAN die to old age, but he cannot be physically harmed. Rtz activates the same general way wonder of u does, if someone tries to harm/kill the user. In my opinion, poisons and diseases should still harm him, but that's up to interpretation. For example: someone tries to trip giorno. Rtz activates. Someone tries to stab him. Rtz activates. But if someone poisons his water in advance, he eats it and may die.
