the fielder coming from second base could have gotten out of the way easily, he just did it too late. This isn't a problem with the rules. The runner cannot run outside the basepath (3ft) to cause the interference without being out himself, so this is ALL on the fielder being lazy/sloppy.

It looked like the runner initiated the contact. Do they review those plays?

The runner can initiate the contact. If the fielder does not have the ball, he has to avoid the runner!

it doesn't matter who initiates contact, the fielder has no right to be in the base path. If the runner runs outside the base path (3ft) before making contact then the runner is out for running outside the base path (line from the runner to the base when the tag attempt begins)
