Southwest can fly the regular 737-700 from HNL to SFO.

My dad used to fly that route for business trips predominantly on the 737-700ERs. He always preferred it over equivalent ANA options on the 787s that would usually service Mumbai or Delhi.

It was used for premium travel, like what BA did with the A318 on the LCY-JFK flights.

It's high. Exponentially higher than using wide body jets. But then again, business tickets are easily one or two thousand dollars per seat.

British Airways also had all-premium cabin A319 aircraft... not sure if they still fly those aircraft.

Because they’re not efficient on anything except the niche they were built for. And if that niche doesn’t work the planes are useless.

@@MrSchwabentierI disagree. Those two planes would have been perfect today for long haul flights by narrow body planes, which many airlines are doing now. The A220 serves such routes today, so why not these specially built B737-700 ER made for ANA?
