Someone it didn’t have anything to do with his spine, it was the knives DIO threw before he died, most hit Kakyoin’s other areas(legs, arms,etc...) but one hit a vital area, his stomach. Kakyoin probably chose not to go to a hospital as well.

Part 5 was already out by the time this game came out (hence Polnareff having Chariot Requim as a brief super). So I think the developers used Kakyoin as an example of fate. That no matter how much the details changed, Kakyoin was always fated to face DIO, be instrumental in defeating the vampire, and die from his injuries.

Guys, I figured out how kakyoin died, near the cutscene when dio is defeated, before kakyoin used emerald splash to kill dio, dio uses his knifes and all 3 of them connected and by the looks of it hit hieroplant green, which if anyone remembers when stands are hit, it also affects the user.

In his dying moments Dio implanted vampric essence into Kakyoin's right lung, hoping to turn him against the Joestars once again as his final and greatest zombie. Thinking it was just a wound he was going to succumb to, Kakyoin prepared for his death but Jotaro convinced him to go to a hospital when they landed in Japan. Iggy and Avdol didn't die for him to perish with Dio. They did a lot of seemingly impossible things on their journey, living on with battle wounds shouldn't be too hard. Kakyoin's infected lung was removed before he could turn into a zombie. But will he join Iggy and Avdol sooner than he expected?
