Thank you for this! Was just having a discussion with someone about doing joint mobilisation. I mentioned to treat the pathological accessory movement through physical exam findings rather than use the kaltenborn rule to determine what accessory movement we need to use to access certain physiological range of motion restrictions. Sure, the rule can be a start, but we should also treat the impairment that we find!

Agree. Only problem with examination of accessory movement has generally a low to max fair reliability, so really hard to say if a joint is hypomobile or not

@@Physiotutors Sorry, I just try to understand it: so let's say I have a pat.with adhesive capsulitis and find a restricted elevation and outer rotation ....what would be your approach then regarding mobilisation of the capsule?

This kind of reminds me of the thinking behind the SSMP (Lewis 2009) and, not directly related, a lot of the skepticism my fellow students and I expressed with regards to "poor" scapular movement during our kinesiology classes. At the end of the day, it seems like you can establish which accessory movements are available at a given joint, and perhaps establish algorithms for what order to try them out for mobilization, but you still have try them out (assuming no contraindication) to see if they help your patient's symptoms, and then move forward from there into training that has a longer term effect.
