@@Aeson-np8tr If this method was truly as routine in physicals as you've asserted then it would not seem as shocking as it does to so many commenters--many of whom are medical professionals and former school athletes, and most of whom have received countless physicals since infancy throughout adulthood. Their actual experience of this method of pulse taking would mitigate their negative response, no? I've certainly never had it done this way in decades of receiving physicals, including in-school high school physicals for athletics. So, I'm also going to go with the reality of my lived experience that its not THAT common. Unfortunately, there is a cultural war currently going on to overtly sexualize school aged children under the guise of "inclusion" and efforts even to rebrand child predators as sympathetic figures called MAPS so I'd prefer that all parents "erred" on the side of being extremely vigilant concerning those given access to their minor children.

I'm a doctor, and I do high school physicals. Never would I ever need to take a groin pulse for any routine exam on otherwise healthy kids. There are numerous other places that are available to conduct this examination such as both wrists, arms, feet, ankles, elbow, and neck. None of those anatomic locations would be considered for most people to be improper or too embarrassing for public contact. There is simply no reason for anyone to routinely go straight there unless it is symptoms directed for a more detailed examination. The fact that the medical establishment came up with some lame excuse regarding an extremely rare condition that this guy could have supposedly screen for in healthy individuals is disappointing for me. It doesn't matter if he's doing this to both sexes. It is an indicator of possibly a deeper problem with this individual that warrants closer examination of his other activities. I do sports physicals as a voluntary contribution to my community outside of my specialty, but this person is a contracted physician and definitely should have known better in sensitive interactions with kids at this phase of their development. Either way, being entrusted to care for patients implies an expectation of high standards. Good job to mom for calling out this behavior!!

With all due respect, I also do such physicals, and the guidelines supported by the American Academy of Pediatrics, American Academy of Family Physicians, American Heart Institute, and others include checking for coarctation this way. Many state/district sports physical exam forms have it listed in the exam as well. You may choose not to follow the guideline, or the people who create the form can choose not to add it (many don't follow all the guidelines), but I don't think it is okay to admonish another provider for simply following the established medical guidelines.

With due respect, guidelines are GUIDES to practices in relation to situations that are encountered. They are not absolute unyielding rules. Kind of like correct writing and grammar for communication. Your typing and composition are difficult to read and follow, but not uncommon in these casual internet exchanges. It’s your prerogative to write in this manner in this situation. These guidelines are intended for a “complete” formal physical often performed in a physician’s office. For a cursory examination in a gymnasium with hundreds of kids throughput over several hours, we have to stick to mostly core exams, and defer more detailed assessments to a formal office visit if irregularities are found or guided by history. Moreover, although the guidelines are endorsed by AAFP, etc, but common sense and common practice are that when a detailed examination of socially sensitive regions of the body such are breasts and genitalia, another person (parent or staff) should be in attendance to maintain a healthy sense of professionalism in exam room. Say what you want , but this situation described doesn’t give the appearance of appropriate interactions at a general sports screening physical. It should never bring about this type of reaction from parents or students. My judgment and common sense will never allow me to provoke such a response regardless of whatever guidelines are proclaimed. Those who have appropriate bedside manner know this to be true, and would certainly handle it tactfully if the examination of this nature is warranted.

As a nurse (30+ years) who has assisted in literally hundreds of sports and other physical exams, I have never seen femoral pulses routinely checked, male or female. They are only checked in newborns, trauma to the lower extremities, or if there is symptomatic reason. In our practice, hernia checks on the males, adolescent or adult, or any exam requiring removal of clothes, always had a wittness present, even if other family are in attendance or the practitioner is same sex. This to protect the patient and the practitioner.

It sounds like a major assumption to say that there wasn't a witness, and there wasn't a symptomatic reason for doing so. Yes, however in this case, if all the doctor did was check the femoral and radial pulse simultaneously, then he followed the guidelines for sports physicals by the American Academy of Pediatrics, American Academy of Family Physicians, American Heart Institute, and others in checking for coarctation of the aorta. It's very easy to look at the sports physical form for many states and see that femoral pulse is listed on the form (for that purpose). What he might be faulted for is maybe not explaining what he was going to do and why. Or maybe he did, idk.

Sports physicals are NOT a new phenomenon and are basically the same a routine office exams; if Mom truly did not know what takes place (especially for MALE exams during which his genitals ARE examined) she should have consulted with the children's father [or the athletic director] ahead of signing the consent form. The area checked [for the daughter] is OUTSIDE the area of her swimsuit for crying out loud. People with moderate aortic stenosis may not notice physical problems which is why it goes undiagnosed. What part of checking for undiagnosed conditions do you people not understand? What part of making sure a child is HEALTHY to play do you not get? This is not for people (kids) with diagnosed conditions; undiagnosed heart conditions are why kids can drop on the field [despite not having noted heart abnormalities off the field]. While this is recognized for girls, it is RARELY mentioned for boys even though every practitioner must be aware of boys’ embarrassment. Parents will often accede to their daughters wishes for gender preference and will prepare their daughters for what to expect; boys are RARELY accorded the same consideration..." The above was provided last year in response to the continuing prevalence of PUBLIC sports physicals whereby BOYS are still examined out-in-the-open in the presence of female school staff and female student athletes (and the girls, who do NOT undergo genitals examination, are allowed screening). It is HARMFUL to boys' emotional development that such flagrant violations of MALE patient modesty/privacy has been widely "normalized" and sociologically accepted; when boys get older and learn that they were not ethically accorded the same protections as girls, they DO get upset/traumatized.

Doctors are speaking out about the importance of a femoral pulse reading in teen athletes. Dr. Scott Rand, Director of the Primary Care Sports Medicine Fellowship with Houston Methodist says they’re crucial for detecting a rare and potentially deadly condition that causes the aorta to narrow, known as coarctation of the aorta. “The femoral pulse exists right in the crease where the hip and the abdomen meet,” Rand said. “I’m going to feel the pulse in your wrist and I’m going to put my other hand on the pulse in your groin.” You must be a very well read, well trained nurse.
@Piink94

@seth3672 Let's take your argument and run with it for a moment, shall we? In most other procedures that require a doctor going underneath a patients clothing, there must be a nurse present in the room. Do you understand why that is? It is to protect both the doctor and the patient from complications of malpractice or accused malpractice. These teenagers obviously weren't given that and I doubt this was routine for ALL of the students or it probably would have been reported before this. Gynecological exams are normal, too, but you are always provided another person in the room to make sure what the doctor is doing is necessary and ethically correct, and any patient has the right to say no to anything. These children weren't given either of those rights. Hope that cleared up your confusion on what makes this feel gross

A spokesperson from a medical clinic in Texas (the Medical Colleagues of Texas) made that statement. It was NOT the Texas Board or other respectable medical entity that made that statement. In fact, and at the time you mentioned (2:27), the newscaster specifically stated that the Texas Board DISAGREED with the Medical Colleagues' statement. It's not hard to see why. If it were true that this was a routine procedure, doctors and nurses outside of this clinic would be getting it done in that area (hence "routine"). But they don't. Just b/c this doctor did it routinely does not mean it was actually an accepted medical practice by the medical field. Additionally, the Medical Colleagues spokesperson stated this procedure was to check for an abnormality. Abnormalities like this have to have a medical justification to go beyond the routine. People can't be checked or tested for everything since there are hundreds of thousands of checks and tests that can be done to check for any number of medical issues. What even merited checking for this congenital defect over possible other medical conditions effecting the heart? Physical exams would get expensive really quick. Furthermore, there would need to be parental/guardian consent, if not prescence, before any private areas of a minor can be examined (unless, under very unusual circumstances, such as if the teenager was of age to be legally separated from parental/guardian care and had the respective legal filing of that ahead of time of this physical). I highly doubt the parents signed off consent on having this check done in the groin area. This mom is 100% in the right. This doctor is another Nasser. Don't forget that Nasser also had others who were on his side--completely allowing him to do what he did--who were in the wrong.

Probably because everything that used to be a parental responsibility has been transferred to the education system over the years. Schools now have to teach children the very most basic things that used to be taught in the home. And also, school funding or budgets are like 100x greater than they were 50 years ago. I understand that there might be times where a medical professional might need to deal with people’s groin area for medical reasons. However, in this situation, since we’re talking about a school doctor checking teenagers, I would say that it would be better to avoid it completely. It seems like common sense. Either find another way to do that part of the exam, or if it can’t be avoided and they absolutely need to, contact the parents first, and make sure the parents and student give informed consent as to what’s being done and why BEFOREHAND. It sounds like he just- went for it. This poor teenage girl was probably shocked and disturbed to have a doctor touch part of her groin out of nowhere. Again, the doctor should be looking to avoid these kinds of situations, since he is working with teenaged girls and boys. Just because he’s a doctors doesn’t mean he get unlimited access to your body, no questions asked. It does make you wonder why he didn’t use better judgment, nobody should leave the doctors office feeling like they were violated. That means there very well could be a problem there.

I’ve had a groin aortic pulse checked once, just once, and it was in an awake sedation procedure placing a tunneled femoral catheter from one side that stopped working, to the other groin. Massive bleeding and difficulty getting the catheter through narrow constricted veins led to them not be able to feel my pulse in my neck, arm or pulse reader thing (I thought because it’s freezing cold in interventional radiology). They ended up taking a femoral pulse on the right side they just pulled the old line out of, and told me before they did because I was already freaking out and distressed over the struggle they had advancing the guidewire and catheter on the other side. I’ve been in ICU and multiple other surgeries and procedures and never had another manual femoral pulse taken, nor had I ever had an ent or paramedic take one. I have three kids, and 2 in sports. Ne’er once have I allowed any kids under 16 in an exam room alone unless my sons asked me to step out or tagged out with dad, and vice versa with our daughter. They know what is and isn’t ok with doctors as well and they know they gave the right to ask questions or decline being touched in certain areas if they feel uncomfortable with it. That’s led to a few walk outs in gynecologists offices with my daughter until she was ready. Only time I’ve ever seen a femoral pulse during a physical or exam-when they were babies. If that doc felt it was necessary on a minor-he should have called mom back (hell mom should’ve been required to be in there, not kept out) and explained reasoning to both mom and child and still required to get consent or move the hell on, especially being a sports physical through the district with kids! The reason was clearly stated in the report. To check for coarctation of the aorta. Checking the femoral pulses is a routine part of assessing the cardiovascular system.

I think that I read somewhere that all doctors that are trained in India take pulses that way. I would be interested to know if the doctor is from India. If so he should be told that taking a pulse at that location is not the norm in this country and can be misunderstood as molestation. I would hate to see a good doctor loose his license over something that he thinks is normal, due to his training. If he is not from India, then that’s just plain creepy and he should be investigated.

This is a traumatizing event for that student. If the doctor was checking for a congenital narrowing of the artery that could lead to death, then there should have been clear communication both in writing and before the exam. The student’s and parents right to refuse was violated. Exams should be done by physician chosen by the parent, not at the school. This way a clear history would indicate if such a procedure were necessary. This is definitely going to result in a lawsuit for both school and doctor.

Pretty sure that palpating the groin/femoral artery is not the best practice standard for determining a congenital narrowing. If there were no indications to investigate such a condition, particularly in this context, it is very odd and merely an excuse to try to avoid pending liability for the malfeasance.

They can less intrusively check a pedal pulse on top of the foot. If they have a problem detecting one there then it would be appropriate to check a femoral pulse. Not to mention a femoral pulse is harder to find than a pedal pulse.

Had a DOT physical where the guy was a bit over the top on the hernia test. I had to have my junk out bare and he sat in a chair and rolled right up to me so it was in his face then had me cough. The test can be done by putting two fingers just above the genitals, approximately where this doctor took the pulse. Holding bare genitals has been used in decades. Never have felt so violated. I can’t say anything because if I refuse, I lose my cdl, thus putting my job and license in jeopardy. One drive was snarky with another dr and the doctor asked about his sleeping and if he was tired. He answered with a snarky remark but his sleep was normal for most. The dr used it to restrict his license and he could no longer driver our big trucks. Now, I do have the option to not go back to that doctor but I will have to pay out of pocket for it, but I won’t have someone fondling my junk with it 6” from their face.

I like how the athletic director went and observed him taking pulses of kids and they were all the same way, in the groin area. So the athletic director thinks that since boys and girls are treated the same while she's watching that everything is okay? The athletic director also needs to be looked at for being "in" on this or being an idiot. 🧐😡👀 Another question is how does she know that he has been doing it this way for years? Did she observe him years ago, or have parents been complaining for years about this and this is what she was parroting back to them? I hope a full investigation is done into the doctor and into the athletic director and the healthcare provider.

Plus, if this was what they do to all the kids, why didn't the girl's brother report this happening to him, as well? So, obviously it isn't happening to everyone, so it's NOT normal procedure. That makes it very suspect.

56, and the same. I've seen some EMTs and nurses in comments say that they have rarely ever used this method in all their careers. When they did, it was usually because they couldn't find a pulse anywhere else. They also said, if you want to find an aortic pulse, for a specific condition, it can also be found in the ankles, or back of the knee, esp. because pulse rates are very strong in teenagers.

Family medicine doctors and pediatricians routinely check groin pulses IN BABIES for coarctation of the aorta. In older children you can find it by comparing blood pressure in the arms to blood pressure in the legs, or by listening for a murmur. Checking groin pulses on a teen is malpractice, unless you have a very good reason to be doing so, and even then, ONLY with a parent present and an explanation of what you are doing and why, and preferably with a provider the teen knows and feels safe with.
