Ketamine, an anesthetic and pain reliever, has rapid-acting antidepressant effects that last 3-7 days after a single dose. Those effects are present even in a large portion of treatment-resistant patients. Efficacy has also been shown in anxiety, bipolar disorder, PTSD, and OCD.

It’s nice to be back! This is the largest project I have worked on so far, as it includes a 250-page review article that goes into much more detail about ketamine’s efficacy in multiple psychiatric conditions, along with its mechanism, safety, background information about depression, comparisons with other drugs, and more. The article is linked in the description. Additional videos will also more comprehensively cover ketamine’s mechanism and its use in psychiatry.

This is the first video since I Subscribed and Bell'd your Channel. You should up your timetable, because hearing about the possibilities of Ketamine (something I had not heard since the House MD season two finale) has renewed my hope in there being a more viable alternative for treating bipolar depression than quetiapine fumerate, which has a nasty side effect of total insomnia if you go off of it for just a couple days.

@@emd344 I was put on K after I died in the ER 3yrs ago, suffering a heart embolism due to the effects of Sepsis had on my body, creating a blood clot in my leg, where two out of three joint replacements had to be removed, leaving me in a wheelchair & using two canes to walk until I am able to have my replacements revised. It has been EXTREMELY challenging. I did not quite understand the severity of my situation until I was slowly weaned off of the meds that kept me in a drug-induced coma for two weeks. When I did come to, I realized that the HELL I went through was indeed a battle, SPIRITUAL in Nature, for my SOUL!!! It was a game changer. Then I found a temporary connection on the street last Spring & started experimenting with dosing. Having a Bipolar diagnosis, suffering from OCD, Depression & Anxiety for over 30years, K brought me out of that doom & gloom thought pattern, however the shit is so damn expensive. I live in the US & am currently residing in NJ but am a native NYer my entire life up until 4years ago. I am now 54 & am fed up with all the psych/pain meds that have me bound & gagged by Big Pharma. The results are in & I am now ready to change the course of events that actually led me to this video. What is in my best interest in finding a Dr. or a Hospital that provides K treatments, that hopefully will be covered under Medicaid(fat chance, huh?)........I appreciate any advice & I apologize for the long reply. Seth, thanks for the upload. Most of the videos uploaded have been docs (mostly anesthesiologists crossing into psychiatrist territory) plugging their infusion clinics. Admittedly intrigued by this as a means for resistant depression for myself. Thanks again! (Edit) and you probably have done more research than the docs pushing this stuff in the high dollar ket clinics. In any case, have to be grateful for the bold ones willing to provide an effective treatment. Hopefully, in time, as we gather more positive outcome data, insurance companies will become willing to reimburse for such treatments.

I think the thing is with the substance is it creates a therapeutic window of remission, in which to process and open up, and engage in compassion focused therapy and open up to positive social experiences in nature. I’d probably lean towards empathogens like mescaline / MDMA used in family group therapy, proffesionly facilitated.

I am not knocking the anti-depressant effects of ketamine or its importance... but isn't it weird how we have to show these chemicals have benefit for the most suffering of populations for it to even be considered to be given to the rest of society? Cannabis for cancer patients, MDMA for PTSD, ketamine for the depressed, and LSD for the near-death? Just legalize, regulate the stuff, and let the adults swim.

I wish the Department of Veteran's Affairs would approve Ketamine for mental health issues. Their reason for not approving its use: They don't want to commit doctors to the time it takes to provide Ketamine therapy. Since patients need to be monitored during infusion and then for a few hours after, they say that's too much time to keep a doctor with one patient.

I started these treatments but my 2nd appointment was awful. I feel he gave me to high of a dose, and insisted on more after I told him that should be enough as I was starting to feel the effects. My anxiety was high before every appointment afterwards and I rescheduled till eventually I just decided it's not for me. These treatments were supposed to be twice a week, 2 hours each at the clinic. I believe he did a total of 7 nasal sprays, 3 in one and 4 in other side. I said enough at 6 and he told me I'm able to have more so I did. He was going to do another but listened when I said no more. Then 2 hours of hell followed after. The effects at least came down quick at the end but I had a dull ache in my forehead for a few days.

I've done lung function test weeks w/o using ketamine and lung function test using ketamine days just a few days before and it's impact on better breathing is remarkable. Ketamine is amazing in that it has several therapeutic benefits. Ketamine also helped me get off Rx opioids years ago. Ketamine is a great medicine if used properly.

I've been seeing a psychiatrist for a few weeks now & he's mentioned using ketamine a few times. I think after watching this video I'm gonna give in and tell him I'm ready to try.

It's confusing to think that most of the science is based off seratonin when dopamine is such a larger player within serious depressive disorders.

I think shirt terms results data can be very misleading. We want to know about remission after 6 months, 2 years etc, not 7 days. I have a remission of depression for 8 hours after 3 glasses of wine! But that’s good little long term therapeutic effects, or maybe a glass or two of wine in good company once a fortnight does have antidepressant effects. For an alcoholic it could be life threatening. It’s so personal, and therapy needs to be personalised, and well rounded.

I had an Appointment with an Anesthesiologist for a Ketamine infusion, but had to cancel, my brother and Sis-in-law abandoned me, I don't drive and the Doctor is about an hour and 15 minutes away.

Since ketamine is such a dirty drug (effects many receptors) it would be very interesting to see the effects of different analogues or targeted NMDA antagonists. That aside, ketamine is a beautiful substance because it has the ability to do so much.

Other NMDAR antagonists and glutamatergic drugs have mostly failed to match ketamine's effect, including memantine, riluzole, MK-801, rapastinel, and lanicemine. I discuss a number of these drugs here: thedrugclassroom.com/video/ketamine-in-psychiatry/#Section_10_Comparison_With_Other_Drugs

Ive been on antidepressants on and off since sept 2018, and been on sertraline since January, i hate it i get no sleep most nights and rarely notice a difference. Ive recreationally used ketamine and id say personally its not made much of a difference. Its fun though

I believe esketamine is what has been approved by FDA for treatment resistant depression. Interestingly, it's more potent than ketamine apparently.
@TheDrugClassroom
@TheDrugClassroom 4 жыл бұрын
Yes, S-ketamine (Spravato) was approved in 2019 as an intranasal formulation for treatment-resistant depression for use with a daily oral antidepressant. From a dose perspective it is more potent, particularly as an anesthetic. It *may* be inferior to R-ketamine for depression and 3/5 clinical trials performed by its maker failed, though it was approved nevertheless. Both S- and R-ketamine are useful and because they have somewhat different mechanisms it may be best to use them together, as with ketamine infusions, though that's hypothetical at the moment as they have not been properly studied against each other. I discuss this here: thedrugclassroom.com/video/ketamine-in-psychiatry/#74_S-Ketamine_or_R-Ketamine

@@TheDrugClassroom I am assuming that they are enantiomers? And wow that's a great write up you have at the link, is this for a psychopharmacology dissertation/thesis?
@TheDrugClassroom
@TheDrugClassroom 4 жыл бұрын
They are enantiomers. S-ketamine has long been viewed as the more active enantiomer because it's ~4x as potent for anesthesia and has a 4x higher affinity for NMDARs, but antidepressant-relevant research has shown a lot of activity with R-ketamine. Still an active area of debate though, no clear answer as to which is 'better.' This is just for TDC, I enjoyed researching the topic so I figured making a larger than normal resource for it would be worthwhile.

@@TheDrugClassroom it's impressive work nonetheless, thanks for everything you do! Oh and one more thing, I had read somewhere that ketamine is a MOR antagonist. Kind of odd if true, especially given it's used for anesthesia.

The bioavailability is much lower, though. Having said that, I personally had a good experience. Despite the big difference in bioavailability, the therapeutic effect is comparable to what I get currently get with IM injections. I probably would have just stuck with Spravato, especially given how much more cost effective it is, but it irritated my nasal passages to the point of nose bleeds and the drip made me a nauseous (both, from what I understand, are rare side effects).

*Seth, can you make a technical video explaining WHY drugs cause the body to adapt to them ? Why the first time is like a christmas tree effect and subsequent doses are never like the first ? Why this happens ? How to reverse tolerance and receptor downregulation ? How to upregulate receptors artificially ? How to make an amphetamine dose effect as it was on the first time ? How to prevent the body to develop this awful tolerance and the pharmacological basis why this happen ? Are there genetic basis for homeostasis or can we upregulate to a new homeostasis set ? Thanks.*
