Ketogenic Dieting May Increase Your Cholesterol: Don’t Panic- Thomas DeLauer… So you might be watching this video because you went on a low carb high fat keto diet and you saw an increase in your cholesterol levels. Maybe your doctor's telling you that you're doing something terribly wrong. Maybe he's telling you you need to get off that ketogenic diet because your cholesterol levels are high. Maybe you're just scared. Maybe you're just concerned as to why they're higher, or maybe you're trying to be proactive and trying to understand why they might get higher. In this video, I'm going to help you understand that higher levels of cholesterol on a ketogenic diet could very well mean a very, very good and rather cool thing.
You see, we have to remember that we're on a ketogenic diet. Everything is surrounding fats. We're eating a lot of fats, we're metabolizing fats, we're going to have more lipids and we're going to have more cholesterols in the bloodstream, plain and simple. We have to remember that. What's interesting is that higher levels of cholesterol on a ketogenic diet are being linked and associated with weight loss. Believe it or not, once weight loss starts and fatty acid metabolism really kicks in, we see an increase in cholesterol. So it usually means that you're having an influx of weight loss occurring at that very point in time.
You see, as fat starts to break down, cholesterol goes up, and it has to do with the fact that triglycerides are mobilized and they end up getting broken down into fuel but the cholesterol just kind of hangs out. The cholesterol doesn't really get burned for fuel. It's not metabolized like that. It's not energy. So what it does, it ends up sitting in the bloodstream, waiting to get recycled by the liver. It's not doing anything good or doing anything bad. It's just elevated and it's usually indicative of the fact that your body is using a little bit more fat for fuel at that very point in time.
So you will find it's a temporary increase and once the weight loss starts to plateau a little bit and starts to slow down, your cholesterol levels will probably start to come down. You've probably seen an increase in both your HDL and your LDL, and it just has to do with the fact that both of them are mobilized. On that note, we have to understand HDL and LDL and the relationship with ketosis, and once you understand this, everything's going to make a lot more sense and I can almost guarantee you're going to come out of this video overall feeling a lot less stressed out about this situation.
First off, HDL and LDL, they're not even cholesterols. They're transport vehicles. They deliver cholesterols. So you've got high density lipoprotein and low density lipoprotein. Low density lipoprotein, the "bad cholesterol" delivers nutrients. It delivers cholesterol, delivers fat and fat-soluble nutrients to the cell. Very important, but for some reason it gets a bad rap. Then we have HDL. HDL's job is to go around and pick up waste particles or used LDL, particles of LDL, and take them back to the liver for recycling. So somewhere along the lines, messages got going to confused. For some reason we think that LDL is bad because it's delivering fat, even though it's totally important. For some reason we think that purely recycling that fat is the only good thing that should be occurring.

Had my lipids done in the 64 hour of a 72 hour fast . Prior to this been on Keto diet for 8 weeks . Doctor called my LDL went up from 96 to 200 and HDL from 200 to 273 . Triglycerides remained low . I’ve lost 20 lbs feel great ton of energy and mental clarity since starting Keto and weekly 24 hour fasts . My Doctors going to want to put me on statins ! Not happening- how can I feel so good yet be in danger of having clogged arteries and heart disease?. I’m sure getting tested during prolong fast increased the values ,doing my own research this is quite common ......now Ill try to explain this to my old school Doctor ! He thinks something bad is going with me - I’m 55 muscular, workout 6 times a week and have abs and feel like 25 again - and he wants me on meds- glad to come across this video and Dr. Deburgs.

I'm in the same situation and had to research this again to feel good about what I am doing. Doc says he never seen Cholesterol was higher than he has ever seen and says I will have a heart attach or stroke. That news was scary! I'm wanting to believe in this new science for sure. Not going on a statin and hoping for the best! Are you still staying the course??? I am 51 with lots of heart disease in the family.

As always complexity made simple. I just got the news from my dr. that my LDL is high and I came here to see if Thomas had something to say about it and BOOOMMM... there it is. Thanks Thomas. I’m down 37lb in 6 months, lost 10%?body fat and put up 4.5lb of muscle on Keto. Super Happy.

Definitely had this happening to me. Started a low carb high fat intermittent fasting diet and I've lost 55 pounds, feel the best i've ever felt and my triglycerides are low and HDL is fine, but my LDL went through the roof and my doctor freaked out on me, scaring the bejesus out of me. I'm gonna do a test of the LDL particles soon with a different practitioner. As mine didn't even mentioned it or cared.

Mine was 275/196 my doctor wants me to take these statins to lower them down to 199/99. I have not taken them yet! I lost 50lbs so far down to 234, going to lose another 60 more to get down to 175. I slimed up! Blood pressure is at 110/68. My muscles are getting toned as I workout 4 times a week! I have a 34 inch waist going to get it down to 30 inches! I have the marker for HLab27 and have Iritis which has damaged my right eye, it is why my eyes suffer because of the inflammation. And because I also have Sciatica so my back causes my left leg pain. The KetoGenetic Lifestyle is helping me maintain a better healthier body, dealing with the inflammation. But my doctor wants me to lower my cholesterol.

Thomas, I love your channel. You always have pertinent information. I started eating a ketogenic diet on Dec 23rd, 2018. Since then, I have lost 50 lbs. Right now I am at 324 and haven't been here since 2004! I've gone down over 2 sizes in pants. Come off of a blood sugar medication and completely resolved my pre-diabetic status. My triglycerides are great as is my ldl-p. My overall cholesterol has come up a bit but that is due to the elevation of more fatty acids being mobilized for energy usage. I just saw my cardiologist YESTERDAY and he was AMAZED at my progress. He suggested to my GP that NO statins were needed and stated that I would soon need to stop my blood pressure medications. I used to be a weight lifter and was always taught to intake HUGE quantities of carbs for strength . I now, and for years, have suffered the consequences of those ill informed actions. I reached a high of nearly 400 lbs! Even while going by the AMA's suggested diabetic diet system which suggests nearly a 40 to 50% overall intake of carbohydrates.. I am now healthier and happier than I have been in YEARS. I have more energy without crashes, more mental clarity, and more focus, than I have had in a very long time. Thank you for giving the science as best you can without going overboard. And THANK YOU for being a great source of the CORRECT information.

You seriously just answered so many questions for me. I had to go into the hospital due to some stroke-like symptoms (I think it was actually a pinched-nerve). They did a battery of tests on me and prescribed me Lipitor. Apparently, my cholesterol is higher than the doctors would like. This is the thing, I started a ketogenic diet and doing HIIT workouts twice a week back in January and I’ve dropped about 20 lbs. I don’t want to be on medication. I was wondering why my numbers were high. Thank you. Seriously, thank you. 😭 I think I’m going to continue (as best as I can, during this pandemic) eating better. Your channel has been such a support for me. I blame you for my addiction to Brazil nuts and seaweed. 😉

I have been on keto diet and intermittent fasting for 3 months and lost 10 kg, feel great in general. But my doctor just rolled his eyes on me, when I mention keto diet and told me I will end up having heart problems. Seriously, this attitude makes me doubt the entire medical system. More people need to hear this information. I’m constantly telling people if they’re going to switch to keto or take mct oil, they need to bring their inflammation down and cut out all the bs. There’s so many potentially dangerous factors that come with this, its insane. Not against keto, just in order for it to be done right and healthy, the individual needs to be incredibly strict.

You can’t take just one value by itself. I had a blood test after one year of keto and 33lb lost. My LDL did increase but so did my so called good cholesterol but it didn’t stop there. My triglycerides dropped. My liver enzymes dropped. My kidney functions were better. My A1c was better. So while in isolation it seemed my LDL did increase but all other markers improved so I believe that overall my insides and actually outside were better and healthier than pre keto. It’s the medical community that just needs to catch-up with new info since they panic since they only look at LDL in isolation

Just because you may not have the knowledge, does not translate to qualified medical practitioners not knowing.

I agree. These were my numbers from last year to now and feel a little better now. This was last years (about a few mths in diet) Cholesterol=136 (low and good) HDL=29 (low and below 40...not good) LDL=84 (low and good) TRIG=123 (low and good) This year (about a year in diet) Cholesterol=194 (50 point jump...eek..still in normal range...but I'm worried) HDL=54 (now normal/above 40) LDL=126 (jumped but still normal) TRIG=70 (lower and better than last years). I'm a conundrum...but started at 245 and now weigh 184. All my other numbers are excellent. I'm just a little nervous...as i need to lose a little more weight...lol

Total is useless because it includes HDL which they like. They still tell you your total is high when HDL is great. LDL-P via NMT with particle size is the one the aware doctor uses. The CAC via CT scan is the only test that tells you if you DO have heart disease and calcs a cardiac calcium score with heart age estimate for you.

Thanks for this video! My son was on keto diet for a month or so then had his wellness check. His blood work came back telling our doctor he has high cholesterol. I tried to explain to him what this video did. His doctor wanted to put him on meds which I immediately said no to. He is 20 years old and instead of the doctor saying to eat more good fats to lower bad cholesterol, they want u to take meds. That's some bs! Anyways, thank you

I just came back from seeing the doc. He was alarmed that the blood panel I had a week ago showed my cholesterol levels “through the roof,” according to him and that I am “severely dehydrated,” because I have a level of 2+ ketones. He wanted me to start taking a statin again. Told him I am on keto, so all that is expected. He was receptive to it, but still wants to send me to a cardiologist (cya imo) because of my history - heart attack 9 years ago at 36. Been off meds for years and will never go back. Glad I found this channel in while doing my research.

That's pretty funny. Good fats. What about good carbs and good protein. If fat was so beneficial it wouldn't increase your cholesterol.

Yep. Happened to me. After one year on keto and losing 22 pounds, annual blood work showed everything in the correct range. Everything except an elevated LDL. Naturally my doctor freaked out; rather than look at the totality of the report, he fixated on this one number. I asked my doctor to do an advanced lipid panel, but he decided he knew better, and all I got was the basic. All the LDL was lumped into one category--BAD. Thanks for explaining the process; I was not really worried, just concerned that my doctor might try to get me on a statin. I think I need a more enlightened doctor.

When I got into the keto diet, my LDL went way up, as did my total cholesterol. My Dr put me on a statin. Is the statin causing harm by keeping the type A LDL from doing the good you described in this video?

I wish you all the best of health. Keto seems cholesterol rich to me and the comments seem to confirm. I am plant based but I added lots of IF or OMAD and little meat.. I eat rice and cut out sugar and bread to lower my sugar. I lost 30 pounds without trying, as my focus was cholesterol and bp, both dropped

I may go back to oatmeal and brown rice. Keto seems to work but I don't like the LDL levels it gives you despite the weight loss. I don't want the doctor prescribing me statins. Picking the right diet is so hard. Now I am reading that oatmeal and brown rice is bad for you. Then what do I eat? I'll take the chance and eat oatmeal and brown rice.

Its sad that we have to educate our physicians. I wanted a Dexa scan to see my body fat index and my doctor was like " I dont think that's appropriate, there is a ton of exposure to radiation" me "ahhh actually doc, i get more radiation in 45 min at the beach than this scan" clueless. If your primary is an older person, I suggest changing to a younger one, they wont be stuck in the 60's education.

my doctor is young but he is not a lipid panel specialist. plus being that KETO is so new he knows what matters in a lipid panel but didn't understand why LDL would go up on KETO. so i really don't blame him because KETO is new and the doctor's are still learning.

Keto isn't new, it has just gained in popularity recently. It's been used since the 1920's to treat epilepsy. Physician's have almost no nutritional education in their formal schooling and so any knowledge in that area must be gained on their own time.

I mean tech tech technically we did not have the ability to achieve the recommended 65% of our total daily intake in the form of carbohydrates until modern agriculture say 20,000 years ago, we have been this form of ourselves (homosapiens) for about 200,000 years and then big foreheads for millions of years before that so really keto is the oldest diet we've ate, I'm sure it was a much higher protein closer to a carnivore diet but the same metabolic pathway. High carb IS the fad diet haha

If you have cardiologist that means you have heart condition, normal healthy folks normally are not seen by specialist. Medical knowledge is collective and goes through tremendous amount of scrutiny. It takes decades amount of data to be published in textbooks as concrete evidence. I would follow the recommendation of your cardiologist given he is MD/DO not NP/PA. Side note I had er physcian who had six packs, completed in triathlons did cross fit ended up having massive heart attack -had elevated LDLs and bad genetics

Glad I found this video. I started low carb diet/keto Oct 2019, after my Dr. informed me at my physical that I was prediabetic & I would need to loose weight & change my diet so I did. (FYI I have always had high blood pressure) My appt back in January 2020 she was so excited of the amount of weight I lost & how I lowered my A1C. This past Oct she was happy that I continued to loose weight but concerned that my cholesterol went up. I was freaking out especially when she told me to eat oatmeal, all I could think of was Carbs! It's been a year since I have had oatmeal but I have been doing it & taking metimucil (OMG the carbs & sugar in this is no bueno :( ) I've lost 43lbs so far (but still Obese) & I am no longer pre-diabetic. I need to do my homework & figure out what is best to do to lower my cholesterol & keep the carbs low & avoid sugar.

Not everyone's numbers go up. Mine have gone down. My total is 201, LDL is 137, HDL is 64, Triglycerides is 52. I had checked Lipoprotien(a) and it is 35, well within range. LDL is a bit high but everything else is great. My physician stopped telling me to not do keto.

michael smith There's no reason to think they're wrong, although the HDL is too low to be healthy imo. With such a low value, it's reasonable to think that the LDL might be the small dense kind.

Huh? Your family is the gauge of what the standard should be? I've seen nothing to suggest 64 is too low. Besides a remnant cholesterol of ZERO, again if that's possible, would mean almost no risk.

Thanks a lot!! I could connect to this one so so much. I have been doing keto from like last 14 months and now my cholesterol has gone way up as compared to before. I have a huge increase in triglycerides levels (from 108 to 175 mg/dl) and LDL (from 110 to 128 gm/dl) and thus the total cholesterol (from 175 to 205 mg/dl) . I got a little worried this time when i saw the blood reports. But now after listening to ur video, i understand why this is happening. I feel much much better now!! Thanks a lot!! Much love and support to you coming all the way from india. Good work bro. One quick question if u can answer. My HDL stayed around the same value, actually decreased a little. ( from 43 to 41). Is that anything i should be concerned about?

@Robnord1

If you are a vaper (as I have been for the last 3 years), I wouldn't be concerned. The tobacco lobby has paid to have rats vaping 24-7 for the last 10 or so years trying to kill them or prove harm with no significant results. : ) There are medical studies online that explain nicotine effects. In the same league as coffee.

Have a feeling he's going to prescribe statins.
