There are many misconceptions about what a 'knee replacement' actually is. What happens during the surgery? How long will a knee replacement last? Do I need a knee replacement? How long does it take to recovery from a knee replacement? What does the evidence say about different technologies used for knee replacement? Find out answers to all these questions and more in this comprehensive educational video.
Dr Matthew Hutchinson is an orthopaedic surgeon with a special interest in knee replacement using 3D patient specific technology.

Sending patient home same day or next day is a crime. My mother had both knees replaced at same time at age 82 and made a full recovery for one reason….she spent a week in hospital receiving daily intensive PT and pain control, then 3 weeks of in-patient rehab with more intensive PT and continuing pain management. All of her equipment, such as walker, cane, bedside commode, passive exercise machine, ice machine, was delivered and paid for by insurance. Today hospitals in US discharge you as soon as you wake from anesthesia and doc allows you about 10 days of strong pain meds. Ridiculous. Patient is pretty much on own to do exercises and fight through pain. The focus is not on patient recovery but on increasing profit for insurance providers. American health care has been broken for a very long time. I am not afraid of the TKR surgery….I am worried about the limited support resources available to me post surgery. Very informative video doctor. Thank you.

I had two total knee replacements at the age of 70. It changed my life. My knees are totally pain free, which they hadn't been for 20 years before my surgery. The operations were easy (I was out for the whole thing). My surgeon and the hospital (HSS) was very clear that physical therapy was key to my complete recovery. Once everything (tendons, ligaments, and muscles) healed and range of motion was restored, i was pain free and have been so ever since (I am 78 years old). I ski as often as I can and am a road cyclist that logged 4700+ miles last year; all knee-pain free. If your surgeon recommends total knee replacement surgery, do it. Best medical decision I've ever made.

@@scottwwsi Don't remember my range of motion before or after. The physical therapist said it was excellent when I finished therapy. I was in constant pain for years prior to surgery. I had a right knee replacement at 58 in 2008 and developed an infection almost one year later to the day. Surgery was more time consuming back then, plus longer hospitalization. I finally had my left knee done a year ago and I walked in and walked out that same morning. Totally different technology.

My doc sez DaVinci robot makes him much more precise in prosthetic fitment. I had my knee done Dec 2021 and now it works great with no pain at all. Used same drug regimen in Texas as Dr. Hutchinson recommends. Went home day after surgery. Took a couple of weeks to get over the surgery, per se. Started PT and rehab after a week. Had great physical therapist, which in my opinion is as important as the surgeon. Made rapid incremental improvement to strength and range of motion, tapering off as time passed. Get a good doc with lots of experience, use the robot, take the drugs as directed, and get a good physical therapist. If you do those things I think you be happy you did it--i know I am. Lost weight. Do the pre-surgery exercises. Continue to lose weight. Do the post surgery exercises and therapy. Walk. Don't sit around. It's gonna hurt. Get off the narcotics as soon as you can. Lose more weight. Your attitude will play the biggest part in your success.

The doctor and PT says I’m doing quite well. I can walk around the house without a cage or cane, pain during the day is manageable with Tylenol. This video helped me with what to expect, realistic timeline for healing as well as doing my part to promote success. I wish you were in the United States. I WISH I had watched this before my recent knee replacement surgery. You, sir, provide detailed knowledge of everything related to knee replacement surgery.. I experienced a tremendous amount of pain afterwards because frankly the doctors goals here is to NOT provide pain medication after a couple of weeks. Completely ludicrous and stalled my initial PT. I finally found a compassionate pain management specialist that provided enough relief for me to get aggressive in my PT. At going on 4 week mark after surgery I am “caught up” in my PT. But the process has been unnecessarily painful. My suggestion to anyone with a low pain tolerance is to get the aftercare sorted out well before surgery. Thank you for this excellent video.

Had a right knee replacement on July25th 2022 . From really day one post - op my knee waned to buckle . Did have manual repositioned several weeks later ,this problems continued . Fluid was drawn from the knee to check for infection , no infection . plans made at present for a revision of my knee in June of 2023 . My knee was I bad shape before surgery was done , did have several falls before surgery, causing me to start using a walker . Some days I can hardly walk because the left knee is have pain due to the right knee not working right . I do have osteoporosis . My surgeon has never did a MRI and making a model of the knee to refer back to when surgery is being done sounds like s great idea . This is the hardest surgery I have ever been thru . And frankly worried about this revision to my right knee ,if this don't work out . I am only 66 year s old ,I'm not wanting the last year to end up as I am now . My surgeon stated that my muscles was very tight back of my right knee and tried to strengthened them the best he could . Back in late November of 2022 I was trying to walk when my knee popped real loud with pain on right side of my right knee , Saw my surgeon and x- rays was done piece of my bone broke ,yes it did depressed me . I do the best I can in worst shape now ,I need everyone to pray for me that I will get better . Wish you was a surgeon in America . My surgeon does knee replacements for many people , but I'm the first of his patients that he has to be revision on . What advise could you give me ?

There is a fear that something unexpected will fail me and I will not be mobile again. Not being able to walk is so scary. I hope my physician turns out to be as careful and thoughtful with me as Dr. Hutchinson is with his patients. They are planning on sending me home on the same day as the surgery and I am living alone. All of this is a bit much for me. Thank you Dr. Hutchinson!! Fantastic description and explanation of TKR. I had mine done 2.5 weeks ago and had a lot of pain in the first 2 weeks. Because of the epidemic here in the US with addiction to Opioids, I was worried about taking them, so I stopped taking them 3 days post op. That was the biggest mistake I made. After being assured I wouldn’t become addicted in the short period of time I would be on them, it was hard to play “catch up” with the pain once it set in, than the first 2 days when I was comfortable on them. So please, take your pain meds as prescribed! I am completely off the pain meds and only take Acetaminophen (Tylenol) occasionally. I go to Physical Therapy 3 times a week and I was told my progress is amazing. So the lessons I learned were : Use your pain meds accordingly, Lots of Ice and Elevation, Rest, movement, and go to Physical Therapy for the best outcome.

Here's my two cents. Before the surgery find out who will provide pain management because I got 10 days of meds and was then told go to a pain management doctor from then on. The meds didn't help at all and I was in some serious frickin pain 24/7 for 3 weeks and then every day after it was about half the day. Mostly at night. I'm not a wimp and it was about as much as I could physically and mentally accept. Right now at 5 weeks I can get around some and pain is tolerable. Good luck!

This video is extremely helpful to me. I am about to have the TKR due to severe OA. The gel shots didn't work, i can't bear full weight or walk or stand with a foot. The pain is excruciating. I had tried all the alternative treatments and it isn't working. So, thank you so much for this invaluable video. Now i am ready. let's do it!!

I wish I’d seen this before my knee replacement. I still have a huge amount of swelling but no pain! My other knee is gradually becoming painful but still manageable. I’ll take your details for reference. Thankyou for doing a great job of educating us.

I’m almost four months post op and my foot and leg are still ridiculously swollen. I was badly bruised after surgery, the foot looked nothing like a foot from swelling. Post op ex rays shows a squarish piece of prosthesis in place there, nothing like what’s shown here. Best part, I woke up mid surgery to hear and feel the hammer blows, saw and drill. Didn’t feel pain because of the nerve block shunt in my thigh, just the blows’ force. It took that long for the anesthesiologist to lift the blanket over my head to see me awake and put me out again! I was outside the hospital waiting for my ride, four hours after waking up. Do your research on your chosen surgeon.

I wish I I had seen this before to give me more information , although I had no alternative, I would have known more about what to expect . I am on day 8 now after the surgery, trying to manage the pain and all the symptoms you mentioned . Again thank you, and now I know what to expect in the future months to come.

Thank you Dr Hutchinson, I am Australian but live in Germany and I am booked to have a TKR with Dr Haas in Bonn in February 2023. I have patellofemoral arthritis and my patellar is bone on bone and laterally displaced. I am 59 and I have been managing with hyalurone injections for 7 years now with my sportsmed orthopod in Bonn, but they are lasting shorter and shorter periods and even doing a 90 minute walk on fairly even not very hilly ground causes pain for days afterwards. Stairs are difficult as well. They recommended a TKR as opposed to a patellar replacement as at my age they have more success with that method. My femoral tibial joint is in reasonable shape apart from osteophytes laterally. My question to you ( as I could not fully understand the German explanation) is: since the patellar is laterally displaced, does that cause changes to the patellar tendon and quadriceps tendon? And if so, how is this addressed with relation to the surgery and rehabilitation?

Thank you Dr Matthew for guiding us on Knee replacement. My main concern was Torn Meniscus show on 25/07/2022 MRI reported Maceration & tear of the body and posterior horn of medial meniscus extrusion 0.2cm. Peripheral tear of the lateral meniscus body with small parameniscal cysts 0.5 to 0.2cm. Partial 50% irregular cartilage ulceration involving the femoral trochlea 1 x 0.5cm & corresponding patellar ridge 1 x 0.7cm without subchondral bone oedema. peripheral patellofemoral osteophytes. Trace effusion in the suprapatellar pouch with non-thicken medial synovial plica. Great that you say not all MRI reports need a knee replacement. Since my fall from boarding the bus in 2020, my knee had been swelling on and off. Ice treatment helps to ease the swelling n pain for short relief. Lately, pain n swelling are more intense unable to go up or down the stairs plus locking weak on walking. Dr recommended going for a full knee replacement. I am thinking about it.

@@ginglyjoe2659 it’s painful to start it took my about 9/10 months to walk downstairs normally upstairs is ok it’s just building the strength back in the quad and hamstring and the calf muscles the arthritic pain may go away, it’s the healing process that gives you the pain and trying to bend the knee as much as you can , sleeping is 50/50 you need to sleep with your legs higher than your body do the swelling , which knee will you have operated on ? I can give you tips on how to get in and out of bed too 👍🏻

Thank you so much Dr. Hutchinson for this very informative video. I greatly appreciate that you give such an in-depth and thorough talk about the topic, pros and cons and all the other peripheral information I will consider other options first before the knee surgery as I have some clicking in the knee but other than that, there’s really no major pain, so I obviously would, or I should say my preference would be to not have surgery anyway thank you so very much again and I will refer this video to anyone who I come into contact with who need some clear and concise answers …you’re the kind of doctor that we all appreciate… You talk in lay terms thank you sir, and God bless

I had a knee replacement 2 weeks ago. I was totally prepared for the operation and exercises BUT not the horrendous pain afterwards. I expected a morphine drip when I woke but apparently this is not the normal. On the second night I spent rocking through the pain sobbing. The next morning the pain specialist came and put a canular into my hand vein and Fentynal for 2.5 days. This allowed me to start working on my exercises. Again I didn't realize the pain levels experienced doing the exercises but I am pushing through as I want full movement to be able to do my wall is again I m happy to have the operation
