With no damage to the threads, I question whether the insert was actually in place at the time of damage.

Agreed. Those inserts are solid, even with the weight of a tank behind the impact. That much deformation just wouldn't have happened.

@@timgosling6189 There’s no way it pushed that insert out. I’d bet a paycheck that the customer tried to screw that insert back in after it was damaged…

It's definitely a head scratcher. @guybeasley2593. I agree with you though. All of us here at the shop believe the insert was not in the valve when the damaged occurred.

Why do you not put wrench, to flats at base of valve?

Hello @toadou8127, that is a great question. Sometimes we do, unless the valve is round. The front and rear of the top of the valve is flat as well, and a safe and suitable option to place the wrench. So most of the time, we just use the top of the valve.
