No, they ARE related. Basically what happens is Kirito got adopted into Suguha's family because his parents died when he was young. His Mother's sister, a.k.a his Aunt, adopted him. Basically Suguha is Kirito's Cousin but since he's adopted that makes Suguha also his adoptive sister.

The anime doesn't cover it like the LN does. So Alfheim runs on the same game engine (cardinal) that SAO ran on. In SAO the character info was stored within the nerve gear. Normally a no-no for MMOs as this can allow for hacked characters but since SAO was a one life death game, the players would never have access and tampering with the headset meant killing the player. Since Kirito worked out a deal with the government guy to keep his nerve gear, he used it to run Alfheim. Also, at the time he dives, his cousin/sister is already in. Since Kirito did a shit load of solo training having been a solo player in SAO, all his skills got incredible amounts of experience. So even though his high level doesn’t matter anymore, his experience skill to skill is still dramatically higher than most because he didn’t have other people doing anything for him while he played before.

Suzy, just to clear things up: both Kirito and Suguha are related by blood; they are first cousins. Kirito was adopted by his aunt (Suguha’s mother). That’s why he’s both legally her cousin and brother.

They said they were brother and sister. But Kirito found out so he stop talking to Sugu. Kirito mom told Sugu that they are cousins after Kirito got stuck in the game. After Kirito cleared the game he started to talk to Sugu again. So Sugu was happy and that is when Sugu started to like Kirito. Kirito got all the same items because they use a backup of SAO to make this game and that is why Asuna is in the game.

Apparently what happened was the game glitching out because two people were logged in from the same location on different console types. So it put the second one to log in right near the first one to log in.

The Japanese version has a little more nuance to it. In that conversation at the end, Kirito asks if it's really okay to kill them. To which Leafa replies of course they were going to do that to you. I think that conversation is an early indicator that Kirito is still carrying a lot of baggage from SAO and he won't exactly play this like a regular player. Fun watch, it's nice to go through this again in English this time and see someone elses reactions.

If I could be in a game, it’d be Dragon Quest, Phantasy Star Online, Fallout 4, or Skyrim, maybe a mix of them. Imagine the world of Dragon Quest with monsters of Dragon Quest as well as Phantasy Star, the combat of Skyrim along with Fallout, the stat and perks of Fallout, and weapons of all of them, while the world is a mix of both sci fi as well as fantasy.

Not only are they cousins by blood, but the whole 'cousin' trope is basically just a way in anime commonly exploited to avoid the 'brothercon' situation and make it look more socially acceptable. But honestly, you can see them as brother and sister, as they were raised to be as much as you and any possible sibling of yours were, and the whole adoption thing is a cop out to make you feel less comfortable about it. That's why he kept his stats(level 80-something stats in a game where regular players *_don't level up_* ), skills, items and money. It's also presumably why he started in the sky of the wrong place, because that was the matching location data to his position in SAO. This gives him something of an unfair advantage, but he doesn't really care right now as long as it helps him save Asuna.
