Feel like you probably wont see this, but the beggar at 4:00 takes 2 trinkets and gives you a random other trinket. The reason it gave you a pill and other pickups is because natural trinkets spawns are replaced by pills it seems

This challenge does actually unlock something, that being the lawn darts item. You probably just had the "unlock all items from the start" fiend folio setting turned on.

Its not saying it but there ARE unlocks for the Fiend Folio challenges, this one unlocks a collectable called Lawn Darts. I recommend trying The Gauntlet, which is both tough on it's own, and has a hidden Badge route where you have to kill the bosses in specific ways. Loving the Fiend Folio content too!

please try out Cadaver's Left Eye from: Cadaver: The Rotten Expansion it fixes your tear rate to 1.5 however all tear are converted into an equivalent damage multiplier (tears cap is still there tho for som reason so maybe having a mod that gets rid of that would be cool!)
