They didn’t fix the pause glitch were you retold twice with the bomb in the shop, I think ur timing is just off. Try to pause just before the bomb explodes and then spam the pause/unpause key until the items have rerolled. Also use unintrusive pause menu

Dangle, The Haunt and Baby Plum seem to be where i Skill Issue the most but where i die the most has to be whenever i get the fallen on the second floor with mid items. Awful experience. Turdlet also gets me quite a bit but far less than others mentioned as at this point i usually have something.
