No, it can be formed with both. Best shown on examples: "Napiš dopis." (Write a letter.) - perfective, meaning "start writing and finish writing, i.e. don't stop until it's done. But on the other hand: "Piš pomalu." (Write slowly.) - imperfective, because there's nothing to complete here. It means (always) write this way. (A more reasonable example would be: "Piš čitelně." (Write legibly.) or "Mluv pomalu." (Speak slowly.)) To say "Open the window" you would normally use the perfective: "Otevři okno." But consider these situations: "Otevři to okno, ale opatrně!" (Open the window, but carefully.) - perfective, because we're referring to a single action, whereas "Otevírej to okno opatrně!" - imperfective, as in "Every time you open this window, do it carefully!" On the other hand, the negative imperative is always imperfective (except some special situations), e.g. "Nezavírej dveře.", "Nepiš (žádný) dopis.", "Nemluv se mnou." Is it clearer now? :)

Nice explanation. However, I am confused when I read a story for beginners and it mixes imperfective and perfective verbs. Here's an example: Vstávám v šest každé ráno (impfc, repeated action). Udělám snídani (why perfective???) a piju kávu (impfc, repeated action) Could you explain please?

The perfective form in the second sentence has the meaning of "futurum exactum" (future perfect; future event predecessing another future event), i.e. first "I make a breakfast" and then "I drink a coffe" - when I drink a coffe, the breakfast is already prepared, that hot coffe is a result of breakfast preparation, which is by that moment completed. If you used the imperfective version, the meaning would change into two concurrent activities - drinking a coffe whereas you are preparing a breakfast, unless you used adverb "potom" or "pak" right after (or instead of) the conjunction "a".

Já dělám, ty děláš, on, ona , ono dělá. My děláme, vy děláte, oni a ony dělají. Budu dělat zítra, dělal jsem včera a všechno jsem podělal - jako pokazil.:-D. My jsme dělali, vy jste dělali, oni a ony dělali nebo budou teprve dělat.

"Dělat" is imperfective. Perfective version is "udělat".

This is not really how it'd work here. There's about 20 000 to 30 000 verbs and nearly all of them have both perfective and imperfective forms so the spreadsheet would be quite long. Even more so again, most of the imperfective verbs have more than one form which slightly changes the meaning too. For example "dělat" which means to do. You have "dodělat" which means to finish something, then you have "předělat" which means to make over in like making a makeover to someone or something. Creating a table would be nearly impossible and I can tell you right now that It doesn't even exit. It's not like English irregular verbs.
