I read somewhere that if you are taking a human npc and you walk in front of a mirror,the npc will see you and count as a witness,that might be why you didn't get silent assassin.

most npcs are unable to become witnesses or find bodies (at least in the tracked stats) when you take them as a human shield. exceptions exist, for example vinnie's wife in A New Life can spot you as you pistol whip her, she has wonky viewcone and will spot you unless you aim upward as you do it. if I had to guess what voided SA in this run, its probably a drug lab workers body being found when the lights come back one. that area is super glitchy and the npcs like to be telepathic. hard to tell without the stat screen though, and even then unconscious bodies that are found, don't show up under the bodies found in stats.

It's funny, the screw-up that knocks Auzzie's rating back to Professional happens right at the end of the level; he runs right into the line of sight of a witness. As for throwing the coin at the door to lure the coke-snorting guards, the coin can rebound towards you if don't throw it right, leading to them seeing you behind the door. A good way to get around that is to just drop the coin outside the door; it'll work just as well is you drop it close enough.
