Is it positively known that the controller was at fault? The military and civilian craft are on different frequencies are an issue. The civilian craft could not hear what was being communicated to the military helicopter. The helicopter has a wider field of vision than the airliner. Is it common for military craft to be doing trading exercises near an active civilian approach? There are so many factors rather than jump on DEI from the get go.
