I think the problem with Kitana and Mileena is they have different views of sisterhood. Jade says Kitana thinks she can be saved, but Mileena and Kitana can't have a relationship like Jade and Kitana because of Sindel and Shao Kahn. I also think that for Mileena being a family means conquering people together, leading wars, taking money from the poor together and Kitana is not about that at all (at least to me from her intros). Their morals and views of sisterhood are different and I think Mileena also sees Kitana as competition and Kitana sees Mileena as a threat which....is understandable from both sides

Also I think they view power differently. Mileena views the role as glamorous and she thinks it belongs to her her rule keeps outworld in line and under her foot similar to the way shao kahn and sindel view it unify under there call. Kitana views it more as a task she must undertake weither she thinks shes worthy or not she believes that the different groups should have a say in the decisions at the top that way it benefits everyone the ruler outworld needs is firm but fair.

I think loyalty and having a place to belong is also a factor here. Kitana HAS a life and friends outside of Shao Kahn and Sindel, even if it's only in the form of Jade as her loyal friend. She has the security of knowing that Jade and Liu Kang etc love her for who she is and not for what she can do to them. This allows for her to worry about the state of the world since her personal sense of self and comfort through a social safety net are all secured. Mileena doesn't have that. Sindel doesn't hide the fact that her love for Mileena is very much conditional. Even Shao Kahn, who ironically is the better parent in that he does care for her as per his MK11 intros, only created her as a replacement to Kitana. As Losty said, had Kitana been loyal to Kahn and his vision, there would be no Mileena, so even her "father's" love comes with that utilitarian condition. Mileena has always felt that, as a clone, her entire identity and even her own right to live is tied to Kitana and her actions rather than her own individual agency. So of course she'd do whatever Shao Kahn wants because he's the only one that shows her ANY form of love that LOOKS unconditional. Her pyramid of needs is not satisfied in that sense. It's also no wonder she fell for Rain's deception, since she's so love-starved even if she herself would never admit it. Thing is, she would have given Edenia to Rain, no questions asked, if she won and he hadn't betrayed her and left Edenia as an independent realm. Mileena IS that loyal, she's the dark side of loyalty given to those who are unworthy and that you won't see their bad sides as bad because you love them and you've normalized their values for yourself.

Losty the difference between kahn and kahnum basically is that kahn is when it is through conquest, kitana had to defeat shao kahn to obtain the title and on the other hand kahnum is when you are the heir to the empire, that's why Mileena claims what she belongs.

The intro where D’Vorah asks Mileena what else she knows of her future, Mileena’s reply does imply she knows D’Vorah kills her so to protect her future (“to protect it”), D’Vorah must die. As for other things (warning: wall of text incoming): Mileena and Kitana are not sisters and cannot be sisters in any way, in fact I don’t even understand why most characters (Kitana herself included) regard Mileena as Kitana’s sister, must be some original timeline notion still subsisting among the writers. Mileena is a clone made from Kitana so their relationship would be more akin to “mother and daughter” since Mileena’s creation did not involve Sindel or Jerrod, let alone Shao Kahn, but rather Kitana’s and some Tarkatan’s (maybe Baraka?) DNAs. Johnny Cage is the only one who actually got it better than everyone else (“so Kitana and Baraka got busy?”). Shao Kahn is not protecting Mileena out of love by banning her from ever using blood magic, on the contrary, he’s making sure she stays useful to him, because that’s all she is to him, a mere pawn. Mileena is a failed experiment and is mentally unstable as seen in MK9, MKX intros (especially Sub-Zero calling her “unstable one”) and MK11, mixing her and blood magic more than likely will drive her already fragile mind into madness, making her useless to Shao. Tanya was more than likely also using Mileena (who did prove to be a very useful idiot) to free Edenia no matter the cost which was her goal, she was going to betray Mileena to Rain and kill whoever remains standing to have the Edenian crown. Tanya is as self-serving and treasonous as they come and anyone in her way is but a means to an end. Believing she truly loved Mileena is buying into Mileena’s own delusion (in MK11 we only see things from Mileena’s perspective) and given her already weak and unstable mind, well… further substantiates this theory. Notice that the affection we see between these two comes from Mileena that vast majority of the time.
