Doctor I'm so glad you are dedicating so much time to solve this problem for those of us who inherited lp(a).

I don't think I'm overly at risk for Lp(a) but when I first got it tested in 2010 my value was 32 mg/dL. It was flagged as high since the upper limit was 30 mg/dL. Back then my diet was OK but not great - eating a fair share of carbs, etc. After reducing carbs and cutting out wheat and grains, my Lp(a) tested consistently in the 15 mg/dL range. More recently, my diet has transitioned a bit more toward higher fat and protein (more animal-based). Some of my recent Lp(a) levels have been as low as 10 mg/dL. I reaIize that my N of 1 experience may not apply to everyone, but it may be worthwhile to consider the role that diet can play with Lp(a). Note: I don't take any statin medications.

lol, it was low to begin with. You’re not making sense. This is for people with actual HIGH LPa, not people like you.

how about some ... NIACIN .. No $$$ to be made

Niacin did not show lowers of cardiovascular end points like myocardial infarct. That's why it's not used more.

So what are you doing at this point? Considering coronary angiogram since calcium score does not identify soft plaque.

@@jakegise7728 A 0 score at 41. I would just keep doing what you are doing. There is so much doctors still don't understand in this area. I personally believe that a high LPa only becomes an issue with other metabolic issues. High blood sugar, blood pressure, inflammation etc.

@@MrAndy9188 Doing the same I suppose. Had a zero CAC at 40 with high LPa.

Niacin lowers Lp(a) 20-30% and Aspirin lowers it 20%. Wonder if the combination will have additive effect; if so, the combination of these cheap drugs will be better than very expensive PCSK9 inhibitors. Does someone know if the combination has been used in some trials.

I thought niacin isn’t that effective and aspirin can cause bleeding so has to be weighed up

I've really got to stop looking at health stuff on the computer, because it scares the heck out of me. I've had health problems, including HBP that responds to medication for about a month, then goes super high, and a recent stroke to the back of the eye with no apparent artery blockage. Now I come across THIS (I have no idea if mine is high or not, but, still). And apparently MOST of what we know about cholesterol should be flushed down the toilet...AAAAAHHHH!

This guy is the uber Lp(a) expert. Used to follow him before I got off Twitter. He will save millions of lives. Brilliant he said the doctors should check their own Lp(a). I had to tell my Yale MD/Phd Cardiologist who I was referred to when I asked for Lp(a) test that he should check his Lp(a). I am on full flush Scientology Niacin at 1000mg. It cut it into half & I started looking like Tom CRUZ. Can’t go any higher, the super flush is a no go. I will stay on it check my insulin resistance & liver enzymes. So far so good 😀

Maybe low levels of Lp(a) are not that good? The Lp(a) levels were inversely correlated with the CIMT in this population, suggesting that subjects with a low Lp(a) level may have a predisposition to carotid atherosclerosis. 2012 article: CIMT in asymptomatic subjects with low Lipoprotein(a) levels.
