Would it be possible to get a comment pertaining to a breach at L5? I started having pain issues and unusual stinging at right side L5 on an L4/L5 fusion at the 6 week post OP point. Dr order xray and then CT and CT shows L5 right side pedicule screw tip is exposed quite significantly (appears about 3/8" but he's not concerned, yet I am having pain issues and went from walking 4 miles a day feeling good about recovery to hurting after just 1 city block of slow walking. I'm very confused about what to do as it was such a sudden change in my recovery?

My surgeon kicked the canyon the road for nearly a year. Once my fusion was complete, he would go in and remove the offending instrumentation on my right side. I insisted on ALL of it being removed so in Sept 2020, on inspection of the fusion during open surgery, the fusion/BMP graft was good so all the instrumentation was taken out.

@@briantargosz3534 Damn....glad they caught it and can correct. I spent almost a whole year waiting for the fusion to stabilize and take. Lots of inflammation in the mean time. Couldn't wait to get it yanked out. I think my body reacted unfavorably to the titanium, that's why I wanted it all out. Good luck with your recovery and do what they tell you, don't blow it during rehab and do things that will jeopardize the procedure! You don't want to start over! 😳

not a good method for the placement of pedicle screws. I learnt to place pedicle screws from C1 to S2 from Professor Harms and Dr Jeszenszky during the end of the 80s and 90s. The Jeszenszky method is by far the best as it consistantly defines the upper outer edge of the pedicle; the rest is thereafter simple. this method was developed for skoliosis surgery and is not affected by hypertrophy or deformity of the facet joints. Jeszenszky has never published his method but I have taught many a house officer in India and in Germany to safely place pedicle screws. Should you wish to know more about this method I woul advise you to contact Dr. D J Jeszenszky at the Schultheiss Klinik In Zurich , where he is the HOD. It is after all his achievement and credit is due unto him.
